小剂量戊酸雌二醇联合微粒化黄体酮胶囊对围绝经期综合征患者Kupperman评分及血清性激素水平的影响

Effects of low-dose estradiol valerate combined with micronized progesterone capsules on Kupperman score and serum sex hormone levels in patients with perimenopausal syndrome

目的探讨小剂量戊酸雌二醇联合微粒化黄体酮胶囊治疗围绝经期综合征的疗效。方法选取2017年6月至2019年10月新乡市第一人民医院围绝经期综合征患者92例,按照随机数字表法分为观察组与对照组,每组46例。观察组采用小剂量戊酸雌二醇联合微粒化黄体酮胶囊治疗,对照组采用微粒化黄体酮胶囊治疗。比较两组疗效:治疗前、治疗3个月后围绝经期症状(Kupperman评分)、血清性激素水平[卵泡刺激素(FSH)、雌二醇(E2)]及骨密度水平(L1~4、双侧股骨)。结果观察组总有效率(95.65%,44/46)高于对照组(82.61%,38/46),差异有统计学意义(P <0.05);治疗3个月后观察组Kupperman评分低于对照组(P <0.05);治疗3个月后观察组血清FSH水平低于对照组,E2水平高于对照组(P <0.05);治疗3个月后观察组L1~4骨密度、双侧股骨骨密度大于对照组(P <0.05)。结论小剂量戊酸雌二醇联合微粒化黄体酮胶囊治疗围绝经期综合征疗效显著,能有效改善围绝经期相关症状,调节血清性激素水平,提高骨密度,可以明显改善患者生活质量。

Objective To investigate the effect of low dose estradiol valerate combined with micronized progesterone capsules on participants with perimenopausal syndrome. Methods From November 2018 to November 2019,a total of 92 patients with perimenopausal syndrome in the First People’s Hospital of Xinxiang were selected and divided into control group and observation group according to random number table method,with 46 patients in each group. The control group was treated with medroxyprogesterone acetate,while the observation group was treated with estradiol valerate combined with micronized progesterone capsules. The two groups were compared in efficacy,symptoms of perimenopausal period before treatment and 3 months after treatment( Kupperman score),serum sex hormone [follicule-stimulating hormone( FSH),estradiol( E2) ] and bone mineral density( L1-4,bilateral femur). Results The total effective rate( 95. 65 %,44/46) of the observation group was higher than that of the control group( 82. 61 %,38/46), and the difference was significant( P < 0. 05),the Kupperman score of the observation group was lower than the control group after 3 months of treatment Group( P < 0. 05). The serum FSH levels in the observation group were lower than those in the control group after 3 months of treatment,and the E2 levels were higher than those in the control group( P < 0. 05). The bone density of L1-4,bilateral femurs in the observation group after 3 months of treatment was greater than that of the control group( P < 0. 05). Conclusions Low dose estradiol valerate combined with micronized progesterone capsule is effective on perimenopausal syndrome,which can improve the symptoms of perimenopausal period,regulate the level of serum sex hormone,increase bone density and significantly improve the quality of life of patients.
