闭合复位弹性髓内针固定治疗儿童肱骨干骨折

Closed reduction and elastic intramedullary nail fixation for treatment of humeral shaft fracture in children

目的探讨闭合复位弹性髓内针固定治疗儿童肱骨干骨折的临床疗效。方法采用闭合复位弹性髓内针固定治疗15例肱骨干骨折患儿。末次随访时,根据Mayo评分标准评价肘关节功能,根据Constant评分标准评价肩关节功能。结果患儿均获得随访,时间6~24个月。骨折均愈合。末次随访时,根据Mayo评分标准评价肘关节功能:优14例,良1例,优良率为15/15;根据Constant评分标准评价肩关节功能:优13例,良2例,优良率为15/15。3例出现针尾激惹症状,取出弹性髓内针后均自行愈合。结论闭合复位弹性髓内针固定治疗儿童肱骨干骨折,手术创伤小,并发症少,术后可较快恢复肩、肘关节功能。

Objective To investigate the clinical effect of closed reduction and elastic intramedullary nailing in the treatment of humeral shaft fracture in children.Methods The 15 children with humeral shaft fractures were fixed with closed reduction and elastic intramedullary nail.At the last follow-up,Mayo and Canstant score criteria were used to evaluate the joint function of the eblow and shoulder,respectively.Results All patients were followed up for 6~24 months.All the fractures were healed.At the last follow-up,the elbow function was evaluated by the Mayo scoring standard:excellent in 14 cases,good in 1,the excellent and good rate was 15/15.According to the Constant score criterion,the shoulder function was evaluated:excellent in 13 cases,good in 2,with excellent and good rate of 15/15.Three cases had irritability symptoms at the nail end,and the symptoms disappeared after the elastic intramedullary needle removal.Conclusions Closed reduction and elastic intramedullary nail fixation for the treatment of humeral shaft fracture in children have satisfactory effect,with less trauma and few complications.The function of shoulder joint and elbow joint can be recovered quickly after operation.
