肺炎支原体感染诱发哮喘患儿外周血miR-33a与miR-222-3p及miR-492表达水平及意义 被引量:5

Expression levels and significance of peripheral blood miR-33a,miR-222-3p and miR-492 in children with asthma induced by Mycoplasma pneumoniae infection

摘要 目的分析肺炎支原体(MP)感染诱发哮喘患儿外周血miR-33a、miR-222-3p、miR-492表达水平及意义。方法选取重庆大学附属三峡医院MP感染患儿345例,依据是否诱发哮喘分为诱发组、未诱发组,比较其外周血miR-33a、miR-222-3p、miR-492表达水平,比较诱发组中不同MP-IgM滴度、哮喘发作分期患儿的miR-33a、miR-222-3p、miR-492表达水平,并分析上述指标与哮喘发作期、MP-IgM滴度的相关性,经Logistic回归法分析MP诱发哮喘的独立危险因素。结果诱发组外周血miR-33a表达水平低于未诱发组,而miR-222-3p、miR-492表达水平高于未诱发组(P<0.05);MP感染诱发哮喘患儿中,MP-IgM高滴度患儿miR-33a表达水平低于低滴度、中滴度患儿,而miR-222-3p、miR-492水平高于低滴度、中滴度患儿,低滴度、中滴度患儿的miR-33a、miR-222-3p、miR-492水平差异也有统计学意义(P<0.05);哮喘急性发作期患儿miR-33a水平低于缓解期患儿,而miR-222-3p、miR-492水平高于缓解期患儿(P<0.05);相关分析发现,MP感染诱发哮喘患儿的miR-33a水平与MP-IgM滴度、哮喘急性发作均呈负相关,miR-222-3p、miR-492水平与MP-IgM滴度、哮喘急性发作均呈正相关(P<0.05);过敏史、家族鼻炎/哮喘史、接受雾化治疗、miR-222-3p、miR-492为MP诱发哮喘的独立危险因素,而miR-33a为保护因素(P<0.05)。结论MP感染诱发哮喘患儿外周血miR-33a呈低表达,而miR-222-3p、miR-492呈高表达,且三者与MP-IgM滴度、哮喘发作分期有一定关联。 OBJECTIVE To analyze the expression levels and significance of peripheral blood miR-33 a,miR-222-3 p and miR-492 in children with asthma induced by Mycoplasma pneumonia(MP)infection.METHODS Total of 345 children with MP infection admitted to Three Gorges Hospital Affiliated to Chongqing University were enrolled in this study.According to the presence or absence of asthma,the children were divided into the induction group and non-induction group.The expression levels of peripheral blood miR-33 a,miR-222-3 p and miR-492 were compared between the two groups.The expression levels of miR-33 a,miR-222-3 p miR-492 in children with different MP-IgM titer at different stages of asthmatic attack in the induction group were compared.The correlation between above indicators and the stage of asthmatic attack and MP-IgM titer were analyzed.Logistics regression analysis was performed to screen independent risk factors for MP-induced asthma.RESULTS The expression level of peripheral blood miR-33 a in the induction group was significantly lower than that in the non-induction group,while expression levels of miR-222-3 p and miR-492 were significantly higher than those in the non-induction group(P<0.05).Among children with asthma induced by MP infection,miR-33 a in children with high MP-IgM titer was lower than that with low and medium titer,while miR-222-3 p and miR-492 were significantly higher than those with low and medium titer.The differences in the levels of miR-33 a,miR-222-3 p and miR-492 between children with low and medium titer were significant(P<0.05).The level of miR-33 a in asthma children during acute attack stage was significantly lower than that during remission stage,while levels of miR-222-3 p and miR-492 were significantly higher than those during remission stage(P<0.05).It was found by correlation analysis that miR-33 a level was negatively correlated with MP-IgM titer and acute asthma attack,while levels of 222-3 p and miR-492 were positively correlated with them(P<0.05).The allergic history,family history of rhinitis/asthma,nebulization treatment,miR-222-3 p and miR-492 were independent risk factors for MP-induced asthma,while high level of miR-33 a was a protective factor(P<0.05).CONCLUSION The miR-33 a is lowly expressed,while miR-222-3 p and miR-492 are highly expressed in children with asthma induced by MP infection.The three factors are related to MP-IgM titer and stages of asthma attack.
