2型糖尿病患者外周血单个核细胞miR-18a、miR-125b相对表达水平与糖脂代谢、炎性因子及胰岛素抵抗的关系研究 被引量:7

摘要 目的检测2型糖尿病(T2DM)患者外周血单个核细胞miR-18a、miR-125b相对表达水平,并分析其与糖脂代谢、炎性因子及胰岛素抵抗的关系。方法选取2019年5月至2020年6月新乡医学院附属商丘市第一人民医院收治的T2DM患者88例作为T2DM组,同期在该院体检的健康志愿者65例作为对照组,检测受试者外周血单个核细胞miR-18a、miR-125b相对表达水平及糖脂代谢[餐后2 h血糖(2 hPG)、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]、炎性因子[白细胞介素(IL)-1β、IL-6、IL-15、TNF-α]指标,并检测空腹胰岛素(FINS),计算胰岛素抵抗指数(HOMA-IR)。采用Pearson相关分析T2DM患者外周血单个核细胞miR-18a、miR-125b相对表达水平与各糖脂代谢、炎性因子指标及HOMA-IR的相关性。结果T2DM组外周血单个核细胞miR-18a、miR-125b相对表达水平及2 hPG、FPG、HbA1c、FINS、HOMA-IR、TC、TG、LDL-C水平均高于对照组,而HDL-C水平低于对照组,差异均有统计学意义(P<0.05)。T2DM组血清IL-1β、IL-6、IL-15及TNF-α水平高于对照组,差异均有统计学意义(P<0.05)。Pearson相关分析结果提示,外周血单个核细胞miR-18a、miR-125b相对表达水平与2 hPG、FPG、HbA1c、FINS、HOMA-IR、LDL-C、IL-1β、IL-6、IL-15、TNF-α呈正相关,与HDL-C呈负相关(P<0.05),但二者与TC、TG无相关性(P>0.05)。结论T2DM患者外周血单个核细胞miR-18a、miR-125b相对表达水平检测可辅助评估T2DM患者机体炎症状态、糖脂代谢紊乱与胰岛素抵抗情况,为治疗T2DM提供依据。 Objective To detect the relative expression levels of miR-18a and miR-125b in peripheral blood mononuclear cells of patients with type 2 diabetes(T2DM),and to analyze their relationship with glucose and lipid metabolism,inflammatory factors and insulin resistance.Methods 88 patients with T2DM who were admitted to Shangqiu First People′s Hospital Affiliated to Xinxiang Medical College from May 2019 to June 2020 were included as the T2DM group,and 65 healthy volunteers who received physical examination in our hospital during the same period were selected as the control group.The relative expression levels of peripheral blood mononuclear cells miR-18a,miR-125b and glucose and lipid metabolism indicators[2 h postprandial blood glucose(2 hPG),fasting plasma glucose(FPG),hemoglobin A1c(HbA1c),total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C)],inflammatory factors indicators[interleukin(IL)-1β,IL-6,IL-15,tumor necrosis factor-α(TNF-α)]of all subjects were tested.Fasting insulin(FINS)was measured,and insulin resistance index(HOMA-IR)was calculated.Pearson linear correlation analysis was used to analyze the correlation between the relative expression levels of miR-18a and miR-125b in peripheral blood mononuclear cells of patients with T2DM and the glucose and lipids metabolism,inflammatory factors and HOMA-IR.Results The relative expression levels of peripheral blood mononuclear cells miR-18a,miR-125b and 2 hPG,FPG,HbA1c,FINS,HOMA-IR,TC,TG,LDL-C in the T2DM group were higher than those in the control group,while HDL-C was lower than the control group,the difference was statistically significant(P<0.05).Serum IL-1β,IL-6,IL-15,and TNF-αin the T2DM group were significantly higher than those in the control group,the difference was statistically significant(P<0.05).Pearson correlation analysis showed that the relative expression levels of miR-18a and miR-125b in peripheral blood mononuclear cells were positively correlated with 2 hPG,FPG,HbA1c,FINS,HOMA-IR,LDL-C,IL-1β,IL-6,IL-15 and TNF-α.There was negative correlation with HDL-C(P<0.05),but no correlation with TC and TG(P>0.05).Conclusion The relative expression levels of miR-18a and miR-125b in peripheral blood mononuclear cells may assist in the assessment of inflammation state,glucose and lipid metabolism disorders and insulin resistance in patients with T2DM,and which can provide basis for the treatment of T2DM.
