糖皮质激素联合鼻内镜手术治疗慢性鼻窦炎30例疗效观察

Glucocorticoid Combined with Endoscopic Sinus Surgery in the Treatment of 30 Cases of Chronic Sinusitis

目的:观察糖皮质激素联合鼻内镜手术治疗慢性鼻窦炎(CRS)的疗效。方法:选择2019年6月至2020年6月于景德镇市第一人民医院治疗的60例CRS患者,按随机数字表法分为手术组与联合组各30例。手术组采用鼻内镜手术治疗,联合组采用糖皮质激素联合鼻内镜手术治疗,术后1个月评估疗效,并随访6个月。比较两组临床疗效、鼻内镜Lund-Kenndey评分、鼻窦CT Lund–Mackay评分、并发症及复发情况。结果:联合组治疗总有效率(RR)高于手术组,治疗后Lund-Kenndey评分及Lund–Mackay评分低于手术组,并发症总发生率及复发率低于手术组,差异有统计学意义(P<0.05)。结论:在CRS患者中采用糖皮质激素联合鼻内镜手术治疗是行之有效的,有利于改善鼻窦状况及黏膜形态,降低并发症及复发率,可作为治疗CRS的优选方案。

Objective:To observe the efficacy of glucocorticoid combined with endoscopic sinus surgery in the treatment of chronic sinusitis(CRS).Methods:Sixty patients with CRS who were treated in Jingdezhen First People's Hospital from June 2019 to June 2020 were selected and divided into operation group and combined group,30 cases in each group,according to the random number table method.The operation group was treated with nasal endoscopic surgery,and the combination group was treated with glucocorticoid combined with nasal endoscopic surgery.The curative effect was evaluated 1 month after the operation,and the follow-up was 6 months.The clinical efficacy,nasal endoscopic Lund-Kenndey score,sinus CT lund-Mackay score,complications and recurrence were compared between the two groups.Results:The total response rate(RR)of the combined group was higher than that of the operation group.After treatment,the Lund-Kenndey score and Lund-Mackay score were lower than the operation group.The total incidence of complications and recurrence rate was lower than that of the operation group.The difference was statistically significant(P<0.05).Conclusion:Glucocorticoid combined with endoscopic sinus surgery is effective in CRS patients.It is beneficial to improve the condition of sinuses and mucosal morphology,reduce complications and recurrence rate,and can be used as a preferred treatment for CRS.
