伽玛刀治疗非小细胞肺癌脑转移的临床疗效及预后分析

Clinical efficacy and prognosis of gamma knife radiosurgery for brain metastases from non-small cell lung cancer

目的:分析运用伽玛刀治疗非小细胞肺癌脑转移的临床疗效及预后。方法:2018年1月-2019年11月收治非小细胞肺癌脑转移患者65例,患者脑转移灶个数为14个,肿瘤直径≤5 cm,所有病灶单纯接受伽玛刀治疗,13个月后复查MRI,评价治疗效果;观察放疗不良反应,评估患者生活质量。结果:65例患者中,完全缓解(CR)10例,部分缓解(PR)39例,稳定(SD)13例,进展(PD)3例,治疗总有效率(CR+PR)为75.38%;治疗后6个月生存率为81.54%;治疗期间出现Ⅱ级放疗不良反应3例,Ⅲ级1例,ⅡⅢ级放疗不良反应发生率为6.15%,未出现Ⅳ级不良反应;治疗后患者生活质量评分高于治疗前,差异有统计学意义(P<0.05)。结论:对非小细胞肺癌脑转移患者运用伽玛刀治疗,安全有效。

Objective:To explore the clinical efficacy and prognosis of gamma knife radiosurgery for brain metastases from non-small cell lung cancer.Methods:From January 2018 to November 2019,65 patients with brain metastasis from non-small cell lung cancer were selected,the number of brain metastases was 1 to 4,and the tumor diameter was less than or equal to 5 cm,all lesions were treated with gamma knife alone,MRI was reexamined after 1 to 3 months to evaluate the therapeutic effect.We observed the adverse reactions of radiotherapy and evaluated the quality of life of patients.Results:Among the 65 patients,10 cases had complete remission(CR),39 cases had partial remission(PR),13 cases were stable(SD)and 3 cases were progressive(PD),the total effective rate(CR+PR)was 75.38%.The 6-month survival rate was 81.54%.During the treatment,there were 3 cases of gradeⅡadverse reactions and 1 case of gradeⅢadverse reactions,the incidence of gradeⅡtoⅢradiotherapy adverse reactions was6.15%,there was no gradeⅣadverse reaction.After treatment,the quality of life score was higher than that before treatment,the difference was statistically significant(P<0.05).Conclusion:Gamma knife radiosurgery is safe and effective for patients with brain metastases from non-small cell lung cancer.
