ANLN在肝内胆管细胞癌和肝细胞肝癌中的差异表达及其在二者诊断中的附加价值

The Differential Expression of ANLN in Intrahepatic Cholangiocarcinoma and Hepatocellular Carcinoma and Its Added Value for Fiagnosis

目的检测Anillin(ANLN)在肝内胆管细胞癌(ICC)和肝细胞肝癌(HCC)样本中的表达差异,并分析ANLN表达与表观扩散系数(ADC)的相关性,评估ANLN在这两种肝癌鉴别诊断中的附加价值。方法选择98例患者为研究对象,其中25例ICC和73例HCC。应用免疫组织化学方法检测手术标本中ANLN表达。所有患者在进行任何抗癌治疗之前均进行了扩散加权磁共振成像(DWI-MRI)检查。ADC值是在Syngo工作站上自动计算。采用Pearson相关检验评估ADC值与ANLN表达的相关性。结果在ICC中,ANLN蛋白免疫组织化学检测阳性率为48.0%(12/25),显著高于HCC的阳性率17.8%(13/73)(P<0.001;Pearson卡方检验)。此外,ANLN阳性表达与ICC组织学亚型和肿瘤分化程度密切相关(P<0.05)。HCC患者的平均ADC值为(1.04±0.15)×10^(-3)mm^(2)/s,ICC患者的平均ADC值为(1.37±0.17)×10^(-3)mm^(2)/s,组间比较差异具有统计学意义(P=0.005)。ANLN表达得分为0、1、2、3、4患者的ADC值分别为(0.96±0.10)×10^(-3)mm^(2)/s、(1.08±0.16)×10^(-3)mm^(2)/s、(1.17±0.13)×10^(-3)mm^(2)/s、(1.35±0.18)×10^(-3)mm^(2)/s和(1.45±0.12)×10^(-3)mm^(2)/s。ADC值与ANLN表达呈显著正相关(r=0.508,P<0.001)。结论ANLN在HCC和ICC中存在差异表达,其表达升高与ICC组织学亚型和肿瘤分化程度密切相关。此外,肝脏肿瘤中ANLN表达水平与ADC值呈正相关。因此,ANLN是一种有价值的辅助诊断生物标志物,可以为DWI对HCC和ICC的鉴别诊断提供更多的附加信息。

Objective To detect the difference of Anillin(ANLN)expression in intrahepatic cholangiocarcinoma(ICC)and hepatocellular carcinoma(HCC)samples,and analyze the correlation between the expression of ANLN and apparent diffusion coefficient(ADC),and evaluate the added value of ANLN for differentiating the two liver tumors.Methods A total of 98 patients treated were selected as the study subjects including 25 ICC and 73 HCC.The expression of ANLN in surgical specimens was detected by immunohistochemistry.All patients performed diffusion-weighted magnetic resonance imaging(DWI-MRI)before any anticancer treatment.The ADC value was automatically calculated on a Syngo workstation.The Pearson correlation test was used to assess the correlation between ADC values and ANLN expression.Results In ICC,the positive rate of ANLN was 48.0%(12/25),which was significantly higher than that of HCC 17.8%(13/73)(P<0.001;Pearson’s chi-squared test).In addition,the positive expression of ANLN was closely related to the histological subtype and tumor differentiation of ICC(P<0.05).The average ADC value of HCC patients was(1.04±0.15)×10^(-3)mm^(2)/s,and that of ICC patients was(1.37±0.17)×10^(-3)mm^(2)/s,the difference was statistically significant(P=0.005).The ADC values of patients with ANLN expression score of 0,1,2,3,4 was(0.96±0.10)×10^(-3)mm^(2)/s,(1.08±0.16)×10^(-3)mm^(2)/s,(1.17±0.13)×10^(-3)mm^(2)/s,(1.35±0.18)×10^(-3)mm^(2)/s and(1.45±0.12)×10^(-3)mm^(2)/s,respectively.ADC value was positively correlated with the expression of ANLN(r=0.508,P<0.001).Conclusion The increased expression of ANLN is closely related to the histological subtype and tumor differentiation of ICC.In addition,the expression of ANLN is positively correlated with ADC value.Therefore,ANLN serves as a valuable biomarker which can provide more additional information for DWI in the differential diagnosis of HCC and ICC.
