来氟米特联合阿仑膦酸钠治疗老年类风湿关节炎继发骨质疏松的临床疗效

Clinical efficacy of leflunomide combined with alendronate in the treatment of osteoporosis secondary to rheumatoid arthritis in the elderly

目的:探究来氟米特联合阿仑膦酸钠治疗老年类风湿关节炎继发骨质疏松的临床疗效。方法:2017年12月-2018年10月收治老年类风湿关节炎继发骨质疏松患者82例,随机分为两组,各41例。参照组应用来氟米特治疗,研究组应用来氟米特联合阿仑膦酸钠治疗,比较两组治疗效果。结果:研究组治疗总有效率明显高于参照组,差异有统计学意义(P<0.05);研究组治疗后肿瘤坏死因子及白细胞介素-6水平均低于参照组,差异有统计学意义(P<0.05);研究组治疗后骨密度指数及骨钙素水平均高于参照组,人抗酒石酸酸性磷酸酶水平低于参照组,差异有统计学意义(P<0.05);研究组治疗后关节疼痛指数及肿胀指数均低于参照组,差异有统计学意义(P<0.05)。结论:老年类风湿关节炎继发骨质疏松患者应用来氟米特联合阿仑膦酸钠治疗,可有效缓解临床症状,提高骨密度,消除炎症因子,提高治疗有效率,具有显著应用价值。

Objective:To explore the clinical efficacy of leflunomide combined with alendronate in the treatment of osteoporosis secondary to rheumatoid arthritis in the elderly.Methods:From December 2017 to October 2018,82 elderly patients with osteoporosis secondary to rheumatoid arthritis were selected and randomly divided into two groups with 41 cases in each group.The control group was treated with leflunomide.The study group was treated with leflunomide combined with alendronate sodium.We compared the treatment effect of the two groups.Results:The total effective rate of the study group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).In the study group,after treatment,TNF and IL-6 levels were lower than those in the control group,the difference was statistically significant(P<0.05).In the study group,after treatment,the BMD index and osteocalcin were higher than those in the control group,and the level of human tartrate resistant acid phosphatase was lower than that in the control group,the difference was statistically significant(P<0.05).In the study group,after treatment,the joint pain index and swelling index were lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion:The application of leflunomide combined with alendronate in the treatment of osteoporosis secondary to rheumatoid arthritis in the elderly can effectively relieve clinical symptoms,improve bone mineral density,eliminate inflammatory factors,improve the treatment efficiency,and has significant application value.
