不同剂量ABO血型血液相容性体外血型血清学变化研究

摘要 目的探究不同剂量ABO血型血液相容性输注后体外血型血清学实验变化。方法体外将患者血标本血液与O型悬浮红细胞及全血进行不同比例混合后,通过试管法与微柱凝胶卡式法ABO血型正反定型,通过聚凝胺法检测血液交叉匹配试验。结果体外模拟A、B、AB型血液与不同剂量O型悬浮红细胞及全血混合后,再次进行ABO血型微柱凝胶法鉴定正定型会出现混合型凝集现象;且随O型悬浮红细胞及全血混合比例增加,其微柱凝胶法正定型混合凝集强度逐渐减弱(P<0.05)。A、B、AB型血液与O型全血混合后,再次进行ABO血型微柱凝胶法反定型会出现与血型不一致的凝集现象;且随O型全血混合比例增加,其微柱凝胶法反定型凝集强度逐渐增强,二者呈正相关(P<0.05)。另外,A型血液与O型全血混合后,再与A型全血、B型血液与O型全血混合后再与B型全血进行交叉匹配试验时,主次侧均出现凝集现象,且随混合比例增加,凝集强度逐渐增强,呈正相关(P<0.05)。结论非O型患者输注O型悬浮红细胞后,对患者再次进行ABO血型正反定型及输注同型血进行血液交叉匹配试验结果影响较小;当患者输注O型全血后,对患者再次进行ABO血型正反定型及输注同型血进行血液交叉匹配试验结果影响较大。 Objective To study the change of blood group serological test in different doses blood of ABO type compatibility after transfusion.Methods Mixed blood samples with O-type suspended red blood cells or blood in different proportion in vitro,then identified positive and reverse ABO blood typing by test tube method and the microcolumn gel cassette assay,did blood cross matching in Polybrene test.Results In vitro,mixed A,B or AB-type blood with different doses of O-type suspension red blood cells or blood,the microcolumn gel cassette assay to identify positive ABO blood typing appeared mixed agglutination phenomenon.Mixed agglutination intensity of positive type in the microcolumn gel test decreased gradually with the mixing ratio of the O-type of suspended red blood cells or blood increased(P<0.05).Mixed the A,B or AB-type blood with different doses of O-type blood,the microcolumn gel cassette assay to identify reverse ABO blood typing appeared mixed agglutination phenomenon because of the blood type not consistent.Mixed agglutination intensity of reverse type in the microcolumn gel test increased gradually with the mixing ratio of the O-type blood increased,they were positive correlation(P<0.05).In addition,mixed A-Type blood with O-type,then A-type blood were used for cross matching test and mixed B-Type blood with O-type,then B-type blood were used for cross matching test,the primary and secondary side could appear agglutination phenomenon,and with the increase of the mixing ratio of O-type blood,the mixed agglutination intensity gradually enhanced.The mixing ratio of O-type blood was positively correlated with the mixed agglutination intensity(P<0.05).Conclusion Patients with non O-type blood infusion with O-type of suspended red blood cells had little effect on positive and reverse ABO blood typing and the same type of blood to do cross matching test,but had a big effect when infusion with O-type blood.
