胆囊结石患者双镜联合保胆取石术后复发的影响因素

Influencing Factors of Recurrence After Bimirror Combined with Choledocholithotomy in Patients with Gallstone

目的探讨胆囊结石患者经腹腔镜联合胆道镜保胆取石术(LCCC)后复发的影响因素。方法选择2019年7月至2020年7月新野县中医院收治的310例胆囊结石患者作为研究对象,患者接受LCCC治疗,术后随访12个月,记录患者随访期间复发情况并分组;询问并记录患者基线资料,分析胆囊结石患者LCCC术后复发的影响因素。结果310例胆囊结石患者经LCCC治疗,术后随访12个月,随访期间41例复发(13.23%),269例未复发(86.77%)。复发组手术时间长于未复发组,饮食油腻、结石数≥3、有胆囊炎急性发作病史患者占比高于未复发组,差异有统计学意义(P<0.05)。logistic回归分析结果显示,手术时间长,饮食油腻、结石数≥3、胆囊炎急性发作病史是胆囊结石患者LCCC后复发的影响因素(OR>1,P<0.05)。结论手术时间长,饮食油腻、结石数≥3、胆囊炎急性发作病史是胆囊结石患者LCCC后复发的影响因素。

Objective To investigate the influencing factors of recurrence in patients with gallstone after laparoscopic combined choledochoscopic choledocholithotomy(LCCC).Methods A total of 310 patients with gallstone treated in Xinye Hospital of Traditional Chinese Medicine from July 2019 to July 2020 were selected as the research subjects.All patients underwent LCCC.The patients were followed up for 12 months,and their recurrence during follow-up was recorded and All patients were divided into groups.The baseline data of patients were asked and recorded,and the influencing factors of recurrence after LCCC in patients with gallstone were analyzed.Results A total of 310 patients with gallstone were treated with LCCC,and they were followed up for 12 months.During the follow-up period,41 cases recurred(13.23%)and 269 cases(86.77%)did not recur.Compared with the non-occurrence group,the operation time was longer,and the proportion of patients with greasy diet,number of calculus≥3 and history of acute cholecystitis was higher in the recurrence group,with statistical significant differences(P<0.05).The results of logistic regression analysis showed that long operation time,greasy diet,number of calculus≥3 and history of acute cholecystitis were the influencing factors of recurrence in patients with gallstone after LCCC(OR>1,P<0.05).Conclusion Long operation time,greasy diet,number of calculus≥3 and history of acute cholecystitis are the influencing factors of recurrence in patients with gallstone after LCCC.
