血脂水平与BMI正常患者子宫内膜癌发生的相关性

Correlation between blood lipid level and endometrial cancer with normal BMI

目的探讨血脂水平与身体质量指数(BMI)正常的患者子宫内膜癌(EC)发生的相关性。方法选取2017年10月至2020年10月海南省人民医院妇科收治的53例BMI正常的EC患者纳入病例组,同期因其他良性疾病收入院术前行诊断性刮宫且BMI正常者55例作为对照组。检测并比较两组患者入院后清晨空腹血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDLC)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A(Apo A)以及载脂蛋白B(Apo B)水平。采用Logistic回归分析判断血脂水平与BMI正常患者子宫内膜癌发病的相关性。结果病例组与对照组患者的TG[(1.58±0.45)mmol/L vs(1.50±0.43)mmol/L]、HDL-C[(1.58±0.30)mmol/L vs(1.55±0.35)mmol/L]与Apo A[(1.54±0.27)g/L vs(1.48±0.47)g/L]比较差异均无统计学意义(P>0.05);病例组与对照组患者的TC[(5.05±0.95)mmol/L vs(4.43±1.34)mmol/L]、LDL-C[(3.32±0.82)mmol/L vs(2.87±0.94)mmol/L]与Apo B[(1.21±0.22)g/L vs(0.90±0.28)g/L]水平比较,病例组明显高于对照组,差异均有统计学意义(P<0.05);经Logistic回归分析结果显示,术前患者的血清TC、LDL-C、Apo B水平与EC发生密切相关(P<0.05),但与HDL-C、TG与Apo A水平无相关(P>0.05)。结论BMI指数正常的子宫内膜癌患者的血清Apo B、TC与LDL-C水平与BMI指数正常的其他良性疾病患者相比存在明显差异性,且相关风险指数高。

Objective To explore the correlation between blood lipid level and endometrial cancer with normal BMI.Methods Fifty-three patients of endometrial cancer with normal BMI(the study group)and 55 patients of gyne-cological benign diseases who had normal BMI and underwent diagnostic curettage before surgery(the control group)were selected from Department of Gynecology,Hainan General Hospital from October 2017 to October 2020.The total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),apolipoprotein A(Apo A),and apolipoprotein B(Apo B)concentration were detected and compared in the morning after admission.Logistic regression analysis was used to determine the correlation between blood lipid level and endometrial cancer in patients with normal BMI.Results There were no significant difference in serum TG,HDL-C,and Apo A among the two groups(P>0.05):TG,(1.58±0.45)mmol/L vs(1.50±0.43 mmol/L;HDL-C,(1.58±0.30)mmol/L vs(1.55±0.35)mmol/L;Apo A,(1.54±0.27)g/L vs(1.48±0.47)g/L.Serum levels of TC,LDL-C,and Apo B were significantly higher in the study group than the control group(P<0.05):TC,(5.05±0.95)mmol/L vs(4.43±1.34)mmol/L;LDL-C,(3.32±0.82)mmol/L vs(2.87±0.94)mmol/L;and Apo B,(1.21±0.22)g/L vs(0.90±0.28)g/L.Lo-gistic regression analysis showed that the serum TC,LDL-C,Apo B level was closely related to the occurrence of EC(P<0.05),but the levels of HDL-C,TG,and Apo A had no correlation with the occurrence of EC(P>0.05).Conclusion Se-rum Apo B,TC,and LDL-C levels in patients of endometrial cancer with normal BMI were significantly different from those in patients of other benign diseases with normal BMI,and the associated risk index was high.
