输血前不规则抗体筛查与鉴定的临床意义分析

Analysis of clinical significance of screening and identification of irregular antibodies before blood transfusion

目的分析输血前不规则抗体筛查与鉴定的临床意义。方法回顾性分析拟在我院输血的22097例住院患者的临床资料,均在输血前使用微柱凝胶法筛查不规则抗体,并对筛查阳性血浆进行抗体特异性鉴定,分析输血史及妊娠史对不规则抗体筛查阳性率。结果在22097例拟输血的患者中,不规则抗体筛查阳性率为0.33%(72/22097);在72例不规则抗体筛查阳性的患者中,有输血史或妊娠史占81.94%;抗体特异性鉴定结果以Rh血型、MNS血型和Lewis血型为主。结论输血前不规则抗体筛查有助于检出具有临床意义的不规则抗体,尤其对有输血史或妊娠史的拟输血的患者,对于提高输血安全性具有积极作用。

Objective To analyze the clinical significance of screening and identification of irregular antibodies before blood transfusion.Methods Clinical data of 22097 inpatients who were to receive blood transfusion in our hospital were retrospectively analyzed.Irregular antibodies were screened by microcolumn gel method before blood transfusion,and antibody specificity was identified on the screened positive plasma-The positive rate of irregular antibody screening was observed by blood transfusion history and pregnancy history.Results The positive rate of irregular antibody screening was 0.33%(72/22097)in 22097 patients to be transfussed.Among the 72 patients with positive irregular antibody screening,77.78%had history of blood transfusion or pregnancy.The results of specific antibody identification were mainly Rh,MNS and Lewis blood groups.Conclusion The screening of irregular antibodies before blood transfusion is helpful for the detection of irregular antibodies with clinical significance,especially for the patients with blood transfusion history or pregnancy history,and has a positive effect on improving the safety of blood transfusion.
