健胃愈脑汤联合尼麦角林治疗气血亏虚型血管性痴呆的临床研究

Clinical study on Jianwei yunao decoction combined with nimergoline in the treatment of vascular dementia of Qi and blood deficiency type

目的评价健胃愈脑汤联合尼麦角林治疗气血亏虚型血管性痴呆的临床疗效。方法选取2017年6月-2020年6月于我院就诊的气血亏虚型血管性痴呆患者90例,按随机数字表法分为两组。两组均予以常规治疗,对照组给予尼麦角林片。在对照组基础上,试验组另给予健胃愈脑汤。比较两组有效率、中医证候评分、血清IL-6、SOD、BDNF、VEGF水平变化及蒙特利尔认知评估量表(MoCA)、日常生活能力量表(ADL)、长谷川痴呆量表(HDS)评分改善情况。结果试验组总有效率明显优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组中医四诊证候评分、血清IL-6、SOD、BDNF、VEGF水平及MoCA、ADL、HDS评分与同组治疗前比较,差异有统计学意义(P<0.05);治疗后试验组中医四诊证候评分、血清IL-6、SOD、BDNF、VEGF水平及MoCA、ADL、HDS评分与对照组比较,差异均有统计学意义(P<0.05)。结论健胃愈脑汤联合尼麦角林治疗气血亏虚型血管性痴呆疗效确切,可以有效改善患者智能水平,值得深入研究及广泛推广应用。

Objective To evaluate the clinical efficacy of Jianwei yunao decoction combined with nicergoline in the treatment of vascular dementia of Qi blood deficiency type.Methods From June 2017 to June 2020,90 patients with vascular dementia of Qi and blood deficiency type were selected and randomly divided into two groups.Both groups were given routine treatment,and the control group was given nicergoline tablets.On the basis of the control group,the experimental group was given Jianwei yunao decoction.The effective rate,TCM syndrome score,serum IL-6,SOD,BDNF,VEGF levels and the improvement of Montreal Cognitive Assessment Scale(MoCA),activities of daily living scale(ADL)and Hasegawa Dementia Scale(HDS)scores were compared between the two groups.Results The total effective rate of the experimental group was significantly better than that of the control group,the difference was statistically significant(P<0.05).After treatment,there were significant differences in TCM four diagnostic syndrome score,serum IL-6,SOD,BDNF,YEGF levels and Moca,ADL,HDS scores between the two groups(P<0.05);after treatment,there were significant differences in TCM four diagnostic syndrome score,serum IL-6,SOD,BDNF,VEGF levels,Moca,ADL,HDS scores between the experimental group and the control group(P<O.O5).Conelusion Jianwei yunao decoction combined with nicergoline is effective in the treatment of vascular dementia of Qi and blood deficiency type,which can effectively improve the intelligence level of patients,and is worthy of further study and wide application.
