基于蒙特卡洛模拟评价比阿培南对肺炎克雷伯球菌连续滴注稳定性研究

摘要 目的通过蒙特卡洛模拟比较比阿培南在治疗肺炎克雷伯菌不同给药方案的药效学,结合比阿培南输液稳定性,优化其给药方案,为临床在治疗肺炎克雷伯菌中提供参考。方法将查阅相关文献,根据比阿培南在健康人群的药代动力学参数,用蒙特卡洛软件模拟10 000例次的治疗方案,得出不同给药方案的达标概率(PTA),并结合比阿培南与常用溶媒配伍后,其12 h内稳定性,优化其给药方案。结果比阿培南在治疗肺炎克雷伯球菌时,单频次给药量相同时,随着频次的增加,PTA逐渐上升,相同给药量,同一频次内,随着滴注时间的上升PTA逐渐上升,且比阿培南在常规生理盐水或5%葡萄糖溶液具有较好的稳定性。结论治疗肺炎克雷伯菌时,增加输液滴注时间,治疗效果较好。 OBJECTIVE To compare the pharmacodynamics of biapenem in the treatment of Klebsiella pneumoniae by Monte Carlo simulation combined with the infusion stability of biapenem, to optimize the drug delivery scheme, and to provide reference for clinical treatment of Klebsiella pneumoniae. METHODS The relevant literature were collected.According to the pharmacokinetic parameters of biapenem in the healthy population, the probability of reaching the standard(PTA) of different drug regimens was obtained by simulating 10 000 treatment regimens with Monte Carlo software.After biapenem was combined with common solvents, its stability within 12 hours was optimized. RESULTS When biapenem was used to treat Klebsiella pneumoniae, PTA increased gradually with the increase of frequency at the same dosage, PTA increased gradually with the increase of infusion time in the same frequency.And biapenem was more stable in normal saline or 5% glucose solution. CONCLUSION In the treatment of Klebsiella pneumoniae, it is better to increased the infusion time.
