间室切除术对舌癌患者吞咽、咀嚼、言语功能及复发的影响

Effect of Mesooecium Resection on Swallowing,Chewing,Speech Function and Recurrence of Patients with Tongue Cancer

目的观察间室切除术治疗舌癌对患者吞咽、咀嚼、言语功能的影响,同时观察患者的复发情况,为舌癌患者手术方案的优化提供参考。方法选取80例舌癌患者为研究对象,采用随机数字表法分为对照组(n=40)及观察组(n=40),2组均接受间室切除术治疗。对照组选择传统下唇裂开进路;观察组选择下颌舌侧松解入路。于术前、术后6个月比较2组吞咽功能[采用标准吞咽功能评价量表(SSA)评估]、咀嚼功能及言语功能[本院自制调查问卷评估];患者术后均随访1年,比较2组并发症发生率及复发率。结果术后6个月,2组SSA评分、咀嚼功能测定值均较术前降低,且观察组低于对照组,差异有统计学意义(P<0.05);术后6个月,观察组言语功能评分高于对照组,差异有统计学意义(P<0.05)。2组并发症发生率及复发率比较,差异无统计学意义(P>0.05)。结论舌癌患者接受间室切除术治疗,并发症少,复发率低,且在下颌舌侧松解入路下进行手术更利于改善患者的吞咽、咀嚼、言语功能,应用价值高。

Objective To observe the effects of mesooecium resection in the treatment of tongue cancer on swallowing,chewing and speech functions of patients,at the same time,to observe the patient’s recurrence,to provide a reference for the optimization of the surgical plan for patients with tongue cancer in the future.Methods 80 patients with tongue cancer were selected as the research subjects.The random number table method was used to divide them into the control group(n=40)and the observation group(n=40),the 2 groups were received mesooecium resection.The control group was used the traditional cleft lip approach;the observation group was used the mandibular lingual release approach.The swallowing function[assessed by the Standard Swallowing Function Assessment Scale(SSA)],chewing function and speech function[assessed by our hospital’s self-made questionnaire]were compared between the 2 groups before and 6 months after operation;patients were followed up for 1 year after operation,and the complication rate and recurrence rate of the 2 groups were compared.Results 6 months after surgery,the SSA scores and measurements of chewing function of the 2 groups were lower than those before operation,and the observation group was lower than the control group,the difference was significant statistically(P<0.05);6 months after surgery,the speech function score of the observation group was higher than that of the control group,the difference was significant statistically(P<0.05).Compared complication rate and recurrence rate between the 2 groups,the difference was not significant statistically(P>0.05).Conclusion Patients with tongue cancer undergoing mesooecium resection treatment has fewer complications and a low recurrence rate.And the operation under the mandibular lingual release approach is more conducive to improving the swallowing,chewing,and speech function of patients,with high application value.
