清气凉营汤联合西医疗法治疗呼吸机相关性肺炎临床研究

Clinical Study on Qingqi Liangying Tang Combined with Western Medicine Therapy for Ventilator-Associated Pneumonia

目的:观察清气凉营汤联合西医疗法治疗呼吸机相关性肺炎(VAP)的临床效果。方法:将68例VAP患者按随机数字表法分为对照组和观察组各34例。对照组给予西医治疗,观察组在对照组基础上加用清气凉营汤鼻饲。比较2组临床疗效、治疗前后降钙素原以及白细胞水平,检测肺功能。结果:观察组总有效率为97.06%,高于对照组82.35%,差异有统计学意义(P<0.05)。治疗前,2组降钙素原以及白细胞水平比较,差异无统计学意义(P>0.05)。治疗后,2组降钙素原以及白细胞水平低于治疗前(P<0.05),观察组降钙素原以及白细胞水平低于对照组(P<0.05)。治疗前,2组肺功能指标比较,差异无统计学意义(P>0.05)。治疗后,2组肺活量(VC)、每分钟最大通气量(MVV)、第1秒用力呼气容积(FEV_(1))高于治疗前(P<0.05),观察组VC、MVV、FEV_(1)高于对照组(P<0.05)。结论:清气凉营汤联合西医疗法治疗VAP患者临床疗效确切,可有效缓解临床症状,改善肺功能。

Objective:To observe the clinical effect of Qingqi Liangying tang combined with western medicine therapy for ventilator-associated pneumonia(VAP).Methods:A total of 68 cases of VAP patients were divided into the control group and the observation group according to the random number table method,34 cases in each group.The control group was treated with western medicine,and the observation group was additionally given the nasal feeding of Qingqi Liangying tang based on the treatment of the control group.The levels of procalcitonin and white blood cells before and after treatment as well as the clinical effect in the two groups were compared;the lung function was detected.Results:The total effective rate was 97.06%in the observation group,higher than that of 82.35%in the control group,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of the levels of procalcitonin and white blood cells between the two groups(P>0.05).After treatment,the levels of procalcitonin and white blood cells in the two groups were lower than those before treatment(P<0.05),and the levels in the observation group were lower than those in the control group(P<0.05).Before treatment,there was no significant difference being found in the comparison of lung function indexes between the two groups(P>0.05).After treatment,the vital capacity(VC),maximum voluntary ventilation(MVV) and forced expiratory volume in the first second(FEV_(1)) in the two groups were higher than those before treatment(P<0.05),and VC,MVV and FEV_(1)in the observation group were higher than those in the control group(P<0.05).Conclusion:Qingqi Liangying tang combined with western medicine therapy for VAP has definite clinical effect,and can effectively relieve clinical symptoms and improve lung function.
