经阴道超声对剖宫产术后瘢痕妊娠的诊断价值分析

Diagnostic value of transvaginal ultrasound in scar pregnancy after cesarean section

目的分析经阴道超声对剖宫产术后瘢痕妊娠(CSP)的诊断价值。方法回顾性选取60例2017年3月至2020年12月于我院就诊的疑诊CSP女性为研究对象,均经腹、阴道超声检查,记录其病理诊断结果及经腹、阴道超声检查与病理诊断结果的诊断符合率,分析经腹、阴道超声检查对CSP的诊断价值。结果经病理检查,60例CSP女性中确诊团块型26例,孕囊型34例。经腹超声检出团块型21例,孕囊型26例,诊断符合率为78%(47/60)。经阴道超声检出团块型25例,孕囊型30例,诊断符合率为92%(55/60)。经阴道超声诊断团块型、孕囊型CSP的灵敏度、特异度、准确率均高于经腹超声。结论经阴道超声与病理诊断结果的诊断符合率及对CSP的诊断价值高于经腹超声。

Objective To analyze the diagnostic value of transvaginal ultrasound in caesarean scar pregnancy(CSP).Methods Sixty suspected CSP women admitted Northwest Women's and Children's Hospital from March 2017 to December 2020 were selected retrospectively.All the patients were examined by transabdominal and vaginal ultrasound,and the diagnostic coincidence rate between transabdominal and vaginal ultrasound and pathological diagnosis were recorded.The diagnostic value of transabdominal and vaginal ultrasound on CSP were analyzed.Results After pathological examination,26 cases with mass type and 34 cases with pregnancy sac type were diagnosed in the 60 suspected CSP women.21 cases with mass type and 26 cases with gestational sac type were detected by transabdominal ultrasound,and the diagnostic coincidence rate was 78%(47/60).25 cases with mass type and 30 cases with gestational sac type were detected by transvaginal ultrasound,and the diagnostic coincidence rate was 92%(55/60).The sensitivity,specificity and accuracy of transvaginal ultrasound in diagnosing mass CSP and gestational sac CSP were higher than those of transabdominal ultrasound.Conclusion The diagnostic coincidence rate with pathological diagnosis and diagnostic value for CSP of transvaginal ultrasound are higher than those of transabdominal ultrasound.
