肩关节镜与传统术式治疗肱骨大结节骨折的优劣差异

Advantages and disadvantages of arthroscopic treatment of humeral greater tuberosity fracture

目的探讨肩关节镜在肱骨大结节骨折临床治疗中优劣差异。方法回顾性分析2016年1月-2019年3月收治的46例肱骨大结节骨折患者的病例资料,按照选择治疗方法不同,将其中23例实施肩关节镜治疗的患者作为观察组,其余23例实施传统手术治疗的作为对照组。观察两组的手术相关情况、肩关节功能恢复情况,并比较两组治疗前、治疗后的肩关节功能评分以及肩关节活动度。结果两组手术相关情况及肩关节功能恢复情况比较差异有统计学意义(P<0.05),观察组显著好于对照组;两组治疗前的肩关节功能评分比较差异无统计学意义(P>0.05),治疗后数据比较中观察组显著好于对照组(P<0.05);两组关节活动度比较差异有统计学意义(P<0.05),观察组显著好于对照组。结论肩关节镜辅助下的半螺纹空心钉内固定手术在治疗肱骨大结节骨折方面的优势明显,可促进肩关节功能的有效恢复。

Objective To study the advantages and disadvantages of shoulder arthroscopy in the treatment of greater tuberosity fracture of humerus.Methods Forty-six patients with humeral greater tuberosity fracture in our hospital from January 2016 to March 2019 were analyzed retrospectively.According to the different treatment methods,23 patients with shoulder arthroscopy were selected as the experimental group,and the remaining 23 patients with traditional surgery were selected as the control group.The operation related conditions and shoulder joint function recovery of the two groups were observed,and the shoulder joint function score and shoulder joint range of motion before and after treatment were compared.Results There were significant differences in operation related conditions and shoulder joint function recovery between two groups(P<0.05).The experimental group was significantly better than the control group,and there was no difference in knee joint function score between the two groups before treatment(P>0.05).After treatment,the experimental group was significantly better than the control group(P<0.05),and the range of motion of two groups was significantly different(P<0.05)The experimental group was significantly better than the control group.Conclusion Arthroscopic assisted half threaded cannulated screw fixation has obvious advantages in the treatment of greater tuberosity fracture of humerus,which can promote the effective recovery of shoulder joint function.
