膝关节全膝置换术围术期自体血回输管理与临床预后相关性分析

Analysis of Perioperative Autologous Blood Transfusion Management and Clinical Prognosis in Total Knee Arthroplasty

目的分析膝关节全膝置换术围术期自体血回输管理与临床预后相关性.方法将78例膝关节全膝置换术患者随机分为研究组和对照组,每组39例,其中,研究组实施围术期自体血回输护理管理,对照组实施常规输血护理管理,对比两组患者凝血功能、免疫功能、并发症发生率及护理满意度等指标.结果术前研究组和对照组患者凝血功能、免疫功能指标比较差异均无统计学意义(P>0.05);术后1 d,研究组患者引流管拔出时间、引流量、术后输血率及术后凝血功能指标、低血压比较差异均无统计学意义(P>0.05),但研究组拆线时间、住院天数及住院费用显著少于对照组(P<0.05);研究组患者免疫功能各指标显著优于对照组(P<0.05);研究组患者对护理服务满意度显著高于对照组(P<0.05).结论膝关节全膝置换术围术期实施自体血回输的护理管理模式,可促进患者免疫功能的恢复,有效预防并发症的发生.

Objective To analyze the relationship between perioperative autologous blood transfusion management and clinical prognosis of total knee arthroplasty.Method78 patients with total knee arthroplasty were randomly divided into study group and control group,with 39 cases in each group,of which the study group performed the whole blood transfusion nursing management during perioperative period,the control group implemented routine blood transfusion nursing management,and the patients in two groups were compared with indicators such as blood coagulation function,immune function,complication rate and nursing satisfaction.ResultsThere was no difference in coagulation function and immune function between the study group and the control group(P>0.05),but there was no difference in drainage tube extraction time,drainage volume,postoperative transfusion rate,postoperative coagulation function index and hypotension complications between the study group and the control group on the 1st day after operation(P>0.05),but the disconnection time,hospitalization days and hospitalization expenses in the study group were significantly shorter than those in the control group(P<0.05).The indexes of immune function in the study group were significantly better than those in the control group(P<0.05),and the satisfaction degree of nursing service in the study group was significantly higher than that in the control group(P<0.05).ConclusionThe nursing management mode of autologous blood transfusion during the perioperative period of total knee arthroplasty can promote the recovery of immune function,effectively prevent the incidence of complications.
