玩偶疗法干预对中重度AD患者精神行为症状、CIBIC-plus评分、认知功能及生活能力的影响

The influence of doll therapy intervention on behavioral and psychological symptoms and CIBIC plus score,cognitive function and viability in patients with moderate to severe AD

目的研究玩偶疗法干预对中重度阿尔茨海默症(AD)患者精神行为症状和印象变化量表(CIBIC-plus)评分的影响。方法选择2018年1月—2020年7月本院收治的60例AD患者为研究对象,随机数表法分为对照组(29例)和观察组(31例)两组。对照组患者给予常规护理,观察组患者在对照组基础上联合玩偶疗法进行护理。比较干预前和干预1个月后,两组患者临床印象变化[印象变化量表(CIBIC-plus)]评分、精神行为症状[痴呆行为量表(BEHAVE-AD)]评分、认知功能[蒙特利尔认知评估(MoCA)]、生活能力[日常生活能力量表(ADCS-ADL)]评分变化。结果干预1个月后,两组患者CIBIC-plus、BEHAVE-AD评分均有所降低,且观察组低于对照组,差异有统计学意义(P<0.05);干预1个月后,两组患者MoCA、ADCS-ADL评分均有所升高,且观察组高于对照组,差异有统计学意义(P<0.05)。结论玩偶疗法干预可以缓解中重度AD患者精神行为症状,改善患者认知功能,减少患者精神行为症状,具有临床推广应用意义。

Objective To investigate the effect of doll therapy intervention on the behavioral and psychological symptoms and clinician interview based impression of change plus(CIBIC plus)score in patients with moderate to severe Alzheimer's disease(AD).Methods A total of 60 patients with AD who were treated in our hospital from January 2018 to July 2020 were enrolled as study subjects,and they were randomly divided into the control group(29 cases)and the observation group(31 cases).Patients in the control group were given routine nursing care,and patients in the observation group were given nursing care based on the control group combined with doll therapy.Clinical interview based impression of change(CIBIC-Plus)score,mental behavior symptom behavioral pathology Alzheimer's disease(BEHAVE-AD)score,cognitive function Montreal Cognitive Assessment(MoCA)score,and life ability Activity of Daily Living Scale(ADCS-ADL)score were compared between the two groups before treatment and 1 month after the intervention.Results After 1 month of intervention,the CIBIC-plus and BEHAVE-AD scores of patients in the two groups were significantly lower than those before intervention,and the observation group was significantly lower than the control group at the same time(P<0.05).MoCA and ADCS-ADL scores of patients in both groups were significantly higher than those before intervention,and those in the observation group were significantly higher than those in the control group at the same time(P<0.05).Conclusions Intervention with doll therapy could relieve the mental and behavioral symptoms of patients with moderate to severe AD,improve their cognitive function,and reduce their mental and behavioral symptoms,which has clinical significance for promotion.
