左乙拉西坦治疗儿童睡眠中癫痫性电持续状态的观察

Observation on the treatment of epileptic electrical status during sleep in children using levetiracetam

目的探究对儿童睡眠中癫痫性电持续状态应采纳左乙拉西坦治疗的临床效果。方法选择2018年6月—2019年5月本院收治的84例儿童睡眠中癫痫性电持续状态患儿作为研究对象,随机数表法分为研究组和对照组两组,每组各42例。对照组采取德巴金治疗,研究组应用左乙拉西坦治疗,对比治疗有效率、脑电图改善、不良反应率。结果研究组较对照组临床发作控制有效率更高(95.24%vs 76.19%)、脑电图改善有效率更高(92.86%vs 76.19%),差异均具有统计学意义(χ^(2)=6.222/4.459,P<0.05)。研究组不良反应率(7.14%)与对照组(14.29%)比较,差异无统计学意义(P>0.05);且两组治疗后语言能、操作能、总智商评分比较,差异无统计学意义(P>0.05)。治疗前两组NSE、NPY水平比较,差异无统计学意义(P>0.05);经治疗后研究组较对照组NSE低、NPY高(P<0.05)。结论在临床儿童睡眠中癫痫性电持续状态(ESES)治疗中应用左乙拉西坦,能增强临床整体疗效,在疾病的发作控制及睡眠期棘波指标改善方面具有显著的价值,且用药安全性高。

Objective To explore the clinical effect of levetiracetam in the treatment of epileptic electrical status during sleep for children.Methods From June 2018 to May 2019,a total of 84 children with epileptic electrical status during sleep were selected as the study subjects.The children were randomly divided into two groups,the study group and the control group,with 42 cases in each group.The control group was treated with Depakine,and the study group was treated with levetiracetam.The effective rate,improvement of EEG and adverse reaction rate(ADR)of the two groups were compared.Results The study group had higher clinical seizure control rate(95.24%vs 76.19%),and higher EEG improvement rate(92.86%vs 76.19%)than the control group,the differences were statistically significant(χ^(2)=6.222/4.459,P<0.05).There was no significant difference in ADR rate between the study group(7.14%)and the control group(14.29%)(P>0.05).There were no statistically significant differences in language ability,operational ability and total IQ scores between the two groups after treatment(P>0.05).Before treatment,there were no significant differences in NSE and NPY levels between the two groups(P>0.05).After treatment,NSE and NPY in the study group were lower and higher than those in the control group,respectively(P<0.05).Conclusions Leveethracetam could enhance the overall clinical efficacy in the treatment of children with epileptic electrical continuous state(ESES)during sleep,and has significant value in the attack control of the disease and the improvement of the spine-wave indexes during sleep,with high safety.
