低分子肝素皮下不同注射速度对皮下出血及疼痛影响的系统性评价

Systematic evaluation of the effects of different injection rates of low molecular weight heparin on subcutaneous hemorrhage and pain

目的:系统评价2种不同速度注射低分子肝素对出血率、皮下出血面积、疼痛度等相关指标的影响。方法:系统检索中英文数据库,主题为低分子肝素皮下注射对照研究。按照纳入和排除标准筛选文献,提取数据并进行质量评价,采用Revman 5.3进行Meta分析。结果:本研究共纳入8项研究,总共1239例患者。其中5篇为英文文献,3篇为中文文献。Meta分析结果显示:低分子肝素缓慢注射法的出血发生率低于快速注射法(RD=-0.23,95%CI:-0.30~-0.16,P<0.01);缓慢注射法的出血面积和直径低于快速注射法(WMD=-2.33,95%CI:-3.69~-0.98,P<0.01);缓慢注射法疼痛度评分低于快速注射组(SMD=-0.38,95%CI:-0.64~-0.13,P<0.01)。结论:低分子肝素缓慢注射法优于快速注射法,可降低出血的发生率、出血面积及VAS疼痛度评分。

Objective:To systematically evaluate the effects of low molecular weight heparin injection at different speeds on subcutaneous hemorrhage and pain degree.Methods:The literatures from Chinese and English databases were systematically retrieved,and the subject was hypodermic heparin injection.According to inclusion and exclusion criteria,the data were extracted,the quality were evaluated,and the meta-analysis was performed by Revman 5.3.Results:A total of 8 studies(5 in English and 3 in Chinese)were included in this meta-analysis,including 1239 patients.The results of meta-analysis showed that the bleeding incidence(RD=-0.23,95%CI:-0.30--0.16,P<0.01),the bleeding area and diameter(WMD=-2.33,95%CI:-3.69--0.98,P<0.01),and the VAS pain score(SMD=-0.38,95%CI:-0.64--0.13,P<0.01)in the slow injection group was lower than those in the rapid injection group.Conclusion:The slow injection of low molecular weight heparin is better than rapid injection,which can reduce the incidence of bleeding,bleeding area and VAS pain score.
