佛山市南海区3~12岁儿童甲状腺功能评估指标参考区间的建立

Establishment of reference interval of thyroid function evaluation index for children aged 3-12 years in Nanhai District of Foshan City

目的:建立佛山市南海区3~12岁儿童甲状腺功能评估指标的参考区间。方法:选取2019年6月至2020年5月南海区3~12岁健康儿童400名,按年龄分为3~6岁组192名,7~12岁组208名。采用化学发光免疫分析仪对2组的总三碘甲状腺原氨酸(total triiodothyronine,TT3)、总甲状腺素(total thyroxine,TT4)、游离三碘甲状腺原氨酸(free triiodothyronine,FT3)、游离甲状腺素(free thyroxine,FT4)、促甲状腺素(thyroid stimulating hormone,TSH)水平进行检测,该指标其呈正态分布,参考区间以数据百分位数数据(P_(2.5)~P_(97.5))表示。结果:2组年龄段参考区间分别为:3~6岁组的参考区间为TT30.59~2.37 ng/ml,TT459.1~115 ng/ml,FT32.08~5.20 pg/ml,FT49.48~17.9 pg/ml,TSH 0.79~5.54μIU/ml;7~12岁组的参考区间为TT30.80~2.57 ng/ml,TT448.1~125 ng/ml,FT31.90~4.94 pg/ml,FT49.77~18.9 pg/ml,TSH 0.49~5.42μIU/ml;不同性别儿童之间甲状腺功能评估指标水平的比较差异无统计学意义(P>0.05);本研究建立的儿童甲状腺功能评估指标参考区间与厂家提供参考区间存在明显不同。结论:本研究建立的甲状腺功能评估指标参考区间有助于南海区3~12岁儿童甲状腺疾病的诊疗与监测。

Objective:To establish the reference interval for the assessment of thyroid function in children aged 3-12 years in Nanhai District,Foshan City.Methods:A total of 400 healthy children aged 3-12 years in Nanhai District from Jun 2019 to May 2020 were selected and divided into 192 children in the 3-6 years old group and 208 children in the 7-12 years old group.Chemiluminescence immunoassay analyzer was used to analyze the levels of total triiodothyronine(TT3),total thyroxine(TT4),free triiodothyronine(FT3),free thyroxine(FT4)and thyroid stimulating hormone(TSH),and they were normally distributed.The reference interval was expressed as a percentile value(P_(2.5)-P_(97.5)).Results:The reference intervals for the two groups were:the reference intervals of the 3-6 years old group were TT3(0.59-2.37)ng/ml,TT4(59.1-115)ng/ml,FT3(2.08-5.20)pg/ml,FT4(9.48-17.9)pg/ml,TSH(0.79-5.54)μIU/ml,and the reference intervals of 7-12 years group were TT3(0.80-2.57)ng/ml,TT4(48.1-125)ng/ml,FT3(1.90-4.94)pg/ml,FT4(9.77-18.9)pg/ml,TSH(0.49-5.42)μIU/ml.There was no statistically significant difference in the levels of thyroid function assessment indexes between children of different genders(P>0.05).The reference interval of thyroid function assessment index for children was significantly different from the reference interval provided by the manufacturer.Conclusion:The reference interval of thyroid function evaluation index established in this study is helpful for the diagnosis,treatment and monitoring of thyroid diseases in children aged 3-12 years in Nanhai district.
