磁共振灌注成像与CT灌注成像在老年缺血性脑血管病患者中的应用价值

The Application Value of Magnetic Resonance Perfusion Imaging and CT Perfusion Imaging in Elderly Patients with Ischemic Cerebrovascular Disease

摘要 目的浅析磁共振灌注成像(PWI)与CT灌注成像(CTPI)对于老年缺血性脑血管患者的应用价值。方法纳入老年缺血性脑血管疾病患者72例,均接受PWI、CTPI检查,就检查方式的检出阳性率与相关灌注参数进行分析。结果CTPI检查与PWI检查相比,阳性检出率显著升高,差异有意义(P<0.05);PWI检查脑梗死患侧及急性脑缺血患侧与健侧相比CBF相对较低,CTPI检查脑梗死患侧与健侧相比,CBF、CBV均明显降低,TTP、MTT指标脑梗死患侧及急性脑缺血患侧与健侧相比显著提升,差异有统计学意义(P<0.05)。结论在老年缺血性血管疾病中,CTPI的相关关注参数变化更大,对病灶的阳性检出率更高,整体诊断效果更佳。 Objective To analyze the application value of magnetic resonance perfusion imaging(PWI)and CT perfusion imaging(CTPI)for elderly patients with ischemic cerebrovascular.Methods 72 elderly patients with ischemic cerebrovascular disease admitted to our hospital were examined by PWI and CTPI,and the positive rate of the examination methods and related perfusion parameters were analyzed.Results Compared with the PWI examination,the positive detection rate of CTPI examination was significantly higher,and the difference was significant(P<0.05).CBF was relatively low on the affected side of cerebral infarction and on the side of acute cerebral ischemia compared with the contralateral side by PWI test.CBF and CBV were significantly lower on both the affected side of cerebral infarction and on the side of acute cerebral ischemia by CTPI test,while TTP and MTT index values were significantly higher on the affected side of cerebral infarction and on the side of acute cerebral ischemia compared with the contralateral side by CTPI test(P<0.05).Conclusions In older patients with ischemic vascular disease,the parameters of interest were more variable for CTPI,with a higher rate of positive detection of lesions and a better overall diagnostic yield.In senile ischemic vascular disease,CTPI's related attention parameters have greater changes,and the positive detection rate of lesions by CTPI is higher,and the overall diagnosis effect is better than PWI.
