两种不同术式治疗卵巢囊肿的临床疗效及对卵巢功能的影响分析

Analysis of the Clinical Efficacy of Two Different Surgical Methods for Ovarian Cysts and Their Influence on Hormone Levels

目的探究经腹及经腹腔镜两种不同术式治疗卵巢囊肿的临床疗效及对卵巢功能的影响。方法选取80例卵巢囊肿患者,按照手术治疗方式的不同分成对照组和治疗组,每组40例。对照组采用传统开腹手术治疗,治疗组采用腹腔镜手术治疗。对比两组临床情况(术中出血量、首次下床活动时间、首次排气时间、住院时间)、性激素水平[抗苗勒管激素(AMH)、促黄体生成激素(LH)、雌二醇(E_(2))、促卵泡生成激素(FSH)、孕酮(P)]、卵巢功能(卵巢体积、卵泡数量)及并发症发生情况。结果治疗组患者术中出血量明显少于对照组,首次下床时间、首次排气时间、住院时间均显著短于对照组,差异有统计学意义(P<0.05)。治疗组LH、FSH水平较对照组显著降低,AMH、E_(2)、P水平较对照组显著升高,差异有统计学意义(P<0.05);治疗组治疗前后卵巢体积、卵泡数量与对照组治疗前后对比,差异无统计学意义(P>0.05)。治疗组并发症发生率为5.00%,显著低于对照组的20.00%,差异有统计学意义(P<0.05)。结论经腹腔镜手术治疗卵巢囊肿,术后恢复快,对卵巢功能影响较小,可降低并发症发生。

Objective To explore the clinical efficacy of two different surgical treatments for ovarian cysts and their influence on hormone levels.Methods 80 patients with ovarian cyst were divided into control group and treatment group according to different surgical treatment methods,with 40 cases in each group.The control group was treated with traditional open surgery,and the treatment group was treated with laparoscopic surgery.The clinical conditions(intraoperative bleeding,time of first getting out of bed,time of first exhaust,length of hospital stay),sex hormone levels[anti Mullerian hormone(AMH),luteinizing hormone(LH),estradiol(E_(2)),follicle stimulating hormone(FSH),progesterone(P)],ovarian function(ovarian volume,number of follicles)and complications were compared between the two groups.Results The amount of intraoperative bleeding in the treatment group was significantly less than that in the control group.The first time out of bed,the first exhaust time and the hospital stay in the treatment group were significantly shorter than those in the control group(P<0.05).The levels of LH and FSH in the treatment group were significantly lower than those in the control group,and the levels of AMH,E_(2) and P were significantly higher than those in the control group(P<0.05);There was no significant difference in ovarian volume and follicle number between the treatment group and the control group before and after treatment(P>0.05).The incidence of complications in the treatment group was 5.00%,significantly lower than 20.00%in the control group(P<0.05).Conclusion Laparoscopic surgery for the treatment of ovarian cysts has a quicker recovery after surgery,has little effect on hormone levels,and can reduce complications and improve prognosis.
