不同时期子宫内膜异位症患者治疗方式及效果评价

Treatment Methods and Effects of Patients with Endometriosis in Different Periods

目的通过分析不同时期子宫内膜异位症患者的治疗方式及其疗效差异,为不同时期子宫内膜异位症患者提供适宜的治疗方案。方法选取156例子宫内膜异位症患者,根据患者不同时期分成四组,分别为青春期组(31例)、育龄期组(65例)、妊娠期组(27例)和围绝经期组(33例),分别应用针对性治疗,比较四组治疗效果。结果围绝经期组患者Ⅰ期患病率(15.15%)低于青春期组(41.94%)、育龄期组(38.46%)、妊娠期组(37.04%),Ⅱ期患病率(30.30%)低于青春期组(41.94%)、妊娠期组(40.74%),Ⅲ、Ⅳ期患病率(30.30%、24.24%)高于青春期组(9.68%、6.45%)、育龄期组(15.38%、9.23%)、妊娠期组(18.52%、3.70%)(P<0.05)。围绝经期组患者保守治疗率低于青春期组、育龄期组、妊娠期组(P<0.05),手术治疗率低于青春期组,高于妊娠期组(P<0.05),药物治疗联合手术治疗率高于青春期组、育龄期组、妊娠期组(P<0.05)。妊娠期组患者保守治疗率高于青春期组、育龄期组(P<0.05),手术治疗率低于青春期组、育龄期组(P<0.05),药物治疗联合手术治疗率低于育龄期组(F<0.05)。育龄期组患者保守治疗率、手术治疗率低于青春期组(P<0.05),药物治疗联合手术治疗率高于青春期组(P<0.05)。四组患者选择不同治疗方式的总有效率比较,差异均无统计学意义(P>0.05)。结论不同时期的子宫内膜异位症患者有不同特征,需要根据情况采取不同的治疗方法才可取得较好的疗效。

Objective By analyzing the treatment methods and curative effects of patients with endometriosis in different periods,we can provide appropriate treatment schemes for patients with endometriosis in different periods.Methods The clinical data of 156 patients with endometriosis were retrospectively analyzed,and the patients were divided into 4 groups according to different periods,namely adolescence group(n=31),childbearing age group(n=65),pregnancy group(n=27)and the pcrimcnopausal group(n=33),respectively applied targeted treatments,and compared the treatment effects of the 4 groups.Results The prevalence of stageⅠ(15.15%)in perimenopausal group was lower than that in adolescent group(41.94%),childbearing age group(38.46%),pregnancy group(37.04%),the prevalence of stageⅡ(30.30%)was lower than that in adolescent gmup(41.94%),pregnancy group(40.74%),and the prevalence of stagesⅢandⅣ(30.30%,24.24%)was higher than that in adolescent group(9.68%,6.45%),childhearing age group(15.38%,9.23%)pregnancy group(18.52%,3.70%)(P<0.05).The rate of conservative treatment in perimenopausal group was lower than that in adolescent group,childbearing age group and pregnancy group(P<0.05),the rate of surgical treatment was lower than that in adolescent group and higher than that in pregnancy group(P<0.05),and the rate of drug treatment combined with surgical treatment was higher than that in adolescent group,childbearing age group and pregnancy group(P<0.05).The conservative treatment rate in pregnancy group was higher than that in puberty group and childhearing age group(P<0.05),the surgical treatment rate was lower than that in puberty group and childbearing age group(P<0.05),and the drug treatment combined with surgical treatment rate was lower than that in childbearing age group(P<0.05).The rate of conservative treatment and surgical treatment in the reproductive age group was lower than that in the adolescent group(P<0.05),and the rate of drug treatment combined with surgical treatment was higher than that in the adolescent group(P<0.05).There was no significant difference in the total effective rates of the four groups(P>0.05).Conclusion Patients with endometriosis in different periods have different characteristics,and different treatment methods need to be adopted according to the situation to achieve better results.
