当归血竭丸加味方治疗白塞病结节性红斑湿热血瘀证疗效观察

Clinical efficacy of modified Danggui Xuejie Pill for nodular erythema of damp-hot and blood stasis in Behcet's disease

目的观察当归血竭丸加味方治疗白塞病(BD)结节性红斑湿热血瘀证的临床疗效。方法将69例BD结节性红斑湿热血瘀证患者按照随机数字表法分为2组。对照组33例予秋水仙碱片口服,治疗组36例予当归血竭丸加味方治疗。2组均治疗2周,并随访3个月。比较2组治疗前后及随访时结节性红斑数量和大小、疼痛视觉模拟评分(VAS)、C反应蛋白(CRP)、红细胞沉降率(ESR)及治疗前后针刺阳性反应情况。结果治疗后及随访时2组结节性红斑个数、大小及疼痛VAS均较本组治疗前降低(P<0.05),随访时2组结节性红斑大小、疼痛VAS及治疗组结节性红斑个数均较本组治疗后降低(P<0.05),且治疗后及随访时治疗组结节性红斑个数、大小均低于对照组同期(P<0.05)。治疗后及随访时2组CRP、ESR均较本组治疗前降低(P<0.05),随访时2组CRP及对照组ESR均较本组治疗后降低(P<0.05)。治疗后2组针刺反应阳性率均较本组治疗前降低(P<0.05),且治疗后治疗组低于对照组(P<0.05)。结论当归血竭丸加味方能改善BD结节性红斑湿热血瘀证患者皮肤症状,改善患者炎性状态,其在减少结节性红斑数量、大小及针刺阳性反应方面优于秋水仙碱,远期疗效佳。

Objective To observe the clinical efficacy of modified Danggui Xuejie Pill for nodular erythema(NE)of damp-hot and blood stasis in Behcet's disease(BD).Methods Totally 69 BD patients with NE of damp-hot and blood stasis were randomly divided into two groups,33 cases in control group were given oral colchicine tablets,and 36 cases in treatment group were given modified Danggui Xuejie Pill.Treated for 2 weeks and followed up for 3 months.The key observation was the number and size of NE,pain visual analogue score(VAS),c reactive protein(CRP),erythrocyte sedimentation rate(ESR),and positive reaction of acupuncture.Results The number and size of EN,pain VAS in groups was decreased after treatment and at follow-up than before treatment(P<0.05);the size of EN and pain VAS in groups at follow-up and number of EN in treatment group at follow-up were decreased than this group after treatment(P<0.05);the number and size of EN in treatment group were lower than those in control group after treatment and during follow-up(P<0.05).CRP in groups was decreased after treatment and at follow-up than before treatment(P<0.05),which in groups were significantly decreased at followed-up than this group after treatment(P<0.05);while ESR in groups was decreased(P<0.05),and which in control group were significantly decreased(P<0.05).Compared with before treatment,positive rate of acupuncture reaction in groups was decreased after treatment(P<0.05),which in treatment group was significantly decreased(P<0.05).Conclusion For BD patients with NE of damp-hot and blood stasis,modified Danggui Xuejie Pill can improve skin symptoms and reduce inflammatory level.It is better than colchicine in reducing number and size of EN,and positive reaction of acupuncture,and the long-term curative effect is better.
