年轻乳腺癌患者改良根治术后身体意象状况及其应对方式的相关性

Study on body image status and coping styles of young breast cancer patients after modified radical mastectomy

目的探讨年轻乳腺癌患者改良根治术后身体意象状况及其应对方式的相关性。方法选取2017年10月—2020年4月医院96例拟行改良根治术治疗的年轻乳腺癌患者作为研究对象,分别于术后3 d(T0)、术后1个月(T1)、术后3个月(T2)、术后6个月(T3)时采用简易应对方式问卷(SCSQ)评估患者术后应对方式,并采用身体意象与关系量表(BIRS)评估患者术后身体意象状况,分析年轻乳腺癌患者改良根治术后身体意象状况及其应对方式的相关性。结果与T0时比较,年轻乳腺癌患者T1、T2、T3时SCSQ问卷中积极应对评分均逐渐提高,消极应对评分均逐渐降低,且BIRS量表中力量和健康维度、社交障碍维度、身体外观和性关系维度评分及总分均逐渐降低,差异均有统计学意义(P<0.05);经双变量Pearson直线相关性分析发现,年轻乳腺癌患者改良根治术后JCS中积极应对评分与BIRS各维度评分及总分均呈负相关(r<0,P<0.05);JCS中消极应对评分与BIRS各维度评分及总分均呈正相关(r>0,P<0.05)。结论年轻乳腺癌患者改良根治术后身体意象状况及其应对方式息息相关,即身体意象状况越佳,患者越容易采取积极的应对方式。

Objective To investigate the relationship between body image and coping style of young breast cancer patients after modified radical mastectomy.Methods A total of 96 young breast cancer patients who planned to receive modified radical mastectomy in the hospital from October 2017 to April 2020 were selected as the research subjects.The postoperative coping styles of the patients were evaluated by the simplified coping style questionnaire(SCSQ)at 3 days of postoperation(T0),1 month of postoperation(T1),3 months of postoperation(T2)and 6 months of postoperation(T3)respectively.The body image and relationship scale(BIRS)was used to evaluate the body image of the postoperation patients,and the correlation between the body image and coping style of the young breast cancer patients after modified radical surgery was analyzed.Results Compared with the T0,T1,T2 and T3 young breast cancer patients in SCSQ in positive coping scores were increased,negative coping scores were reduced gradually,and the BIRS of strength and health dimension,social dimension,physical appearance and sex relationship dimension score and total score were gradually reduced,the diff erence was statistical significance(P<0.05);Bivariate Pearson linear correlation analysis showed that positive coping score in JCS was negatively correlated with BIRS score and total score after modified radical surgery(r<0,P<0.05).Negative coping score in JCS was positively correlated with BIRS score and total score in all dimensions(r>0,P<0.05).Conclusion The body image status and coping styles of young breast cancer patients after modified radical mastectomy are closely related,that is,the better the body image status is,the more likely the patients are to adopt positive coping styles.
