环氧合酶2和CD147在甲状腺癌组织的表达及其临床意义

Expression of cyclooxygenase 2 and CD147 in thyroid carcinoma tissues

目的探讨环氧合酶2(cyclooxygenase 2,COX2)蛋白和CD147蛋白在甲状腺癌组织中的表达及其作用。方法收集2013年2月至2020年12月大庆市龙南医院(齐齐哈尔医学院第五附属医院)病理学确诊的117例甲状腺肿瘤患者的组织标本,其中90例甲状腺癌组织、27例甲状腺瘤组织,另外选择20例正常甲状腺组织作为对照组。采用免疫组织化学法检测组织中COX2蛋白和CD147蛋白的表达水平,进行χ^(2)检验统计学分析两者的表达与甲状腺癌临床病理学参数之间的关系。结果正常甲状腺组织、甲状腺瘤组织和甲状腺癌组织中COX2蛋白表达率分别为5.00%(1/20)、33.33%(9/27)和62.22%(56/90),两两之间比较差异有统计学意义(χ^(2)=7.020、21.461、5.507,P<0.05);COX2表达水平与甲状腺癌TNM分期、颈淋巴结转移、包膜浸润明显相关(χ^(2)=6.328、5.424、4.061,P<0.05),差异有统计学意义。正常甲状腺组织、甲状腺瘤组织和甲状腺癌组织中CD147蛋白表达率分别为10.00%(2/20)、37.04%(10/27)和71.11%(64/90),两两之间比较差异有统计学意义(χ^(2)=10.374、25.463、4.417,P<0.05);CD147表达水平与甲状腺癌TNM分期、组织学分级、颈淋巴结转移和包膜浸润明显相关(χ^(2)=6.110、6.402、5.945、4.787,P<0.05),差异有统计学意义。结论 COX2蛋白和CD147蛋白过表达在甲状腺癌的发生发展及侵袭转移中起重要作用,检测COX2和CD147有助于判断甲状腺癌转移潜能和预后。

Objective To investigate the expression of cyclooxygenase 2(COX2)protein and CD147 protein in thyroid carcinoma,and their effect on thyroid carcinoma.Methods The tissue samples of 117 patients with thyroid tumor pathologically confirmed from February 2013 to December 2020 on Daqing Longnan Hospital(Qiqihar Medical School No.5 Affiliated Hospital)were collected,including 90 cases of thyroid carcinoma tissues and 27 cases of thyroid benign tumor tissues,and 20 cases of thyroid normal tissues were selected as the control group.Immunohistochemical method staining for the paraffin sections(SP method)was used to assess the expression level of COX2 protein and CD147 protein in thyroid normal tissues,thyroid benign tumor tissues and thyroid carcinoma tissues.The statistical analysis of chi-square test was carried out in combination with clinicopathological features of thyroid carcinoma.Results The positive expression rate of COX2 protein in thyroid normal tissues,thyroid benign tumor tissues and thyroid carcinoma tissues were 5.00%(1/20),33.33%(9/27)and 62.22%(56/90)with significant difference(respectively,χ^(2)=7.020,21.461,5.507,P<0.05).The expression level of COX2 was significantly correlated thyroid carcinoma to tumor node metastasis stage(TNM stage),neck lymph node metastasis,capsular invasion(respectively,χ^(2)=6.328,5.424,4.061,P<0.05).The positive expression rate of CD147 protein in thyroid normal tissues,thyroid benign tumor tissues and thyroid carcinoma tissues was 10.00%(2/20),37.04%(10/27)and 71.11%(64/90)with significant difference(respectively,χ^(2)=10.374,25.463,4.417,P<0.05).The expression level of CD147 was significantly correlated thyroid carcinoma to TNM stage,histology grade,neck lymph node metastasis and capsular invasion(respectively,χ^(2)=6.110,6.402,5.945,4.787,P<0.05).Conclusion The overexpression of COX2 protein and CD147 protein in thyroid carcinoma may play important roles in the tumorigenesis,invasion and metastasis of thyroid carcinoma.Detection of the expression levels of COX2 protein and CD147 protein in thyroid carcinoma tissues can help predict the potential capacity of metastasis and prognosis of thyroid carcinoma.
