关节镜下微骨折术治疗膝关节软骨损伤的效果观察及有效率分析

摘要 目的探讨关节镜下微骨折术治疗膝关节软骨损伤的效果与有效率。方法研究对象均为方便选取2018年10月—2020年9月该院收治的膝关节软骨损伤患者94例,以随机数字表法划分为两组。研究组行关节镜下微骨折术,对照组行关节清理术。比较两组患者治疗的总有效率,术前与术后3个月时美国特种外科医院(HSS)膝关节功能评分、Tegner膝关节运动功能评分、膝部关节液炎症因子水平[白细胞介素-1β(IL-1β)与肿瘤坏死因子-α(TNF-α)],以及术后并发症情况。结果两组治疗的总有效率比较中,研究组为95.74%较对照组为80.85%更高,差异有统计学意义(χ^(2)=5.045,P<0.05)。术后3个月时,研究组HSS评分(76.5±11.80)分、Tegner评分(4.8±0.50)分较对照组(65.6±10.50)分、(3.2±0.40)分更高,差异有统计学意义(t=4.731、17.131,P<0.05)。术后3个月时,研究组IL-1β(9.2±2.00)pg/mL、TNF-α水平(10.2±2.80)pg/mL较对照组(12.0±2.20)、(14.3±2.50)pg/mL更低,差异有统计学意义(t=6.456、7.488,P<0.05)。研究组术后并发症发生率为4.25%与对照组的8.51%比较,差异无统计学意义(χ^(2)=0.178,P>0.05)。结论膝关节软骨损伤患者应用关节镜下微骨折术治疗能够获取到满意的效果,进一步改善了患者的膝关节功能与运动功能,安全可靠,适于临床推广。 Objective To investigate the effect and efficiency of arthroscopic microfractures in the treatment of knee cartilage injuries.Methods The 94 study subjects were all patients with knee cartilage injury who were admitted to the hospital convenience selection from October 2018 to September 2020.They were divided into two groups by a random number table.The study group underwent arthroscopic microfracture surgery,and the control group underwent joint debridement.To compare the total effective rate of treatment between the two groups of patients,the United States Hospital for Special Surgery(HSS)knee function score,Tegner knee motor function score,and knee joint fluid inflammatory factor levels before and 3 months after surgery[IL-1β(IL-1β)and tumor necrosis factor-α(TNF-α)],and postoperative complications.Results In the comparison of the total effective rate of treatment between the two groups,95.74%of the study group was higher than 80.85%of the control group,the difference was statistically significant(χ^(2)=5.045,P<0.05).At 3 months after surgery,the HSS score(76.5±11.80)points and Tegner score(4.8±0.50)points of the study group were higher than those of the control group(65.6±10.50)points and(3.2±0.40)points,the difference was statistically significant(t=4.731,17.131,P<0.05).At 3 months after the operation,the levels of IL-1β(9.2±2.00)pg/mL and TNF-α(10.2±2.80)pg/mL in the study group were compared with those in the control group(12.0±2.20)pg/mL,(14.3±2.50)pg/mL was lower,the difference was statistically significant(t=6.456,7.488,P<0.05).The incidence of postoperative complications in the study group was 4.25%,compared with 8.51%in the control group,the difference was not statistically significant(χ^(2)=0.178,P>0.05).Conclusion The application of arthroscopic microfracture treatment for patients with knee cartilage injury can achieve satisfactory results,further improve the knee joint function and motor function of the patient,it is safe and reliable,and suitable for clinical promotion.
