流行性感冒病毒感染与冠心病心血管风险事件发生率的相关性研究 被引量:4

Correlation between influenza virus infection and incidence of cardiovascular risk events of coronary heart disease

摘要 目的探讨流行性感冒病毒感染与冠心病心血管风险事件发生的关系。方法将462例冠心病患者按是否合并流行性感冒病毒感染分为感染组与非感染组,比较两组3个月内心血管风险事件发生率;并按是否发生心血管风险事件分为风险事件组与非风险事件组,比较两组临床特征及流行性感冒病毒感染率的差别,单因素及多因素回归分析筛选冠心病心血管风险事件发生的危险因素。结果感染组总心血管风险事件发生率为29.82%,全因死亡12.28%,高于非感染组的7.90%、1.23%(P<0.05)。单因素及多因素分析:吸烟、HR、合并高血压、合并高血脂、冠脉病变程度、流行性感冒病毒感染、C反应蛋白(CRP)及N末端B型利钠肽前体(NT-proBNP)水平均为影响冠心病近期心血管风险事件发生的危险因素(P<0.05),接种流感疫苗为其保护因素。结论流行性感冒病毒感染与吸烟、HR、高血压、高血脂、冠脉病变程度、CRP、NT-proBNP同为冠心病近期心血管风险事件发生的影响因素,为改善近期预后,必须积极控制以上危险因素,积极接种流感疫苗,以降低冠心病患者心血管风险事件发生风险。 Objective To explore the relationship between influenza virus infection and the incidence of cardiovascular risk events of coronary heart disease(CHD).Methods Four hundred and sixty-two patients with CHD were divided into the infected group and the non-infected group according to the complication with influenza virus infection.The incidences of cardiovascular risk events in both groups within 3 months were compared.According to occurrences of cardiovascular risk events,the subjects were divided into the risk event group and non-risk event group to compare the clinical features and incidences of influenza virus infection in both groups.Univariate and multivariate regression analyses were used to screen the risk factors of cardiovascular risk events of CHD.Results The overall incidences of cardiovascular risk events and all-cause deaths in the infection group were 29.82% and 12.28% and were higher than 7.90% and 1.23%in the non-infection group,respectively(P<0.05);Univariant analysis and multivariant factor analysis showed that smoking,HR,hypertension,hyperlipidemia,the degree of coronary artery disease,influenza virus infection,levels of CRP and NT-proBNP were the risk factors affecting recent cardiovascular risk events of CHD(P<0.05).Influenza vaccination was a protective factor.Conclusions Influenza virus infection and smoking,HR,hypertension,hyperlipidemia,degree of coronary lesions,CRP,NT-proBNP were the influencing factors of the recent cardiovascular risk events of CHD.In order to improve the short-term prognosis,the above risk factors must be actively controlled.It is necessary to actively receive influenza vaccination to reduce the risk of cardiovascular risk events in patients with CHD.
