呈贡区内镜下消化道癌筛查情况及影响因素分析

Analysis of endoscopic gastrointestinal cancer screening and its influence factors in Chenggong district

目的分析呈贡区内镜下消化道癌筛查情况及影响因素。方法采取横断面研究,根据便利抽样法选取2018年12月至2020年12月云南省呈贡区4486名居民作为研究对象,收集其一般资料及疾病资料,并行胃肠镜及病理学检查。采用单因素分析及多因素Logistic回顾分析探讨影响消化道癌发生的因素。结果4486名研究对象中,阳性病例77例,检出率为1.7%,其中男性41例,女性36例。单因素分析结果显示,两组年龄、吸烟史、饮酒史、水果摄入、蔬菜摄入、饮食规律性、幽门螺杆菌比较,差异具有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,年龄、吸烟史、饮酒史、饮食规律性、幽门螺杆菌是影响消化道癌发生的危险因素(P<0.05)。结论年龄大、有吸烟及饮酒史、饮食不规律、幽门螺杆菌阳性人群更易发生消化道癌。针对此类人群,相关部门应做好消化道癌预防知识的宣传教育工作。

Objective To analyze the endoscopic gastrointestinal cancer screening and its influence factors in Chenggong district.Methods A cross-sectional study was conducted.According to the convenient sampling method,4486 residents in Chenggong district,Yunnan province from December 2018 to December 2020 were selected as the research objects.Their general data and disease data were collected,and gastroenteroscopy and pathological examination were performed.Univariate analysis and multivariate Logistic regression analysis were performed to exploring the factors affecting the occurrence of gastrointestinal cancer.Results Among the 4486 research objects,77 cases were positive,and the detection rate was 1.7%,including 41 males and 36 females.The univariate analysis showed that there were significant differences in age,smoking history,drinking history,fruit intake,vegetable intake,dietary regularity and Helicobacter pylori between the two groups(P<0.05).The multivariate Logistic regression analysis showed that age,smoking history,drinking history,dietary regularity and Helicobacter pylori were the risk factors affecting the occurrence of gastrointestinal cancer(P<0.05).Conclusion People with older age,smoking and drinking history,irregular diet and Helicobacter pylori positive are more likely to develop gastrointestinal cancer.For such people,relevant departments should do a good job in the publicity and education of gastrointestinal cancer prevention knowledge.
