标准大骨瓣减压术对重型颅脑损伤患者脑代谢指标及神经功能的影响

Effects of standard large bone flap decompression on cerebral metabolic indexes and neurological function in patients with severe craniocerebral injury

目的探讨标准大骨瓣减压术对重型颅脑损伤患者脑代谢指标及神经功能的影响。方法将96例重型颅脑损伤患者根据治疗方案的不同分为对照组和观察组,各48例。对照组采取常规骨瓣开颅术治疗,观察组采取标准大骨瓣减压术治疗。比较两组的临床效果。结果治疗后,两组的GCS、NIHSS及APACHE-Ⅱ评分均改善,GFAP、S100β及MBP水平均降低,CCP、CEO2、SVO2均升高,且观察组优于对照组(P<0.05)。观察组的术后并发症总发生率低于对照组,恢复良好率及GOS评分均高于对照组(P<0.05)。结论标准大骨瓣减压术治疗重型颅脑损伤患者的效果显著,能有效改善患者的神经功能、脑代谢指标及相关血清指标水平,降低术后并发症发生率,提升患者预后。

Objective To explore the effects of standard large bone flap decompression on cerebral metabolic indexes and neurological function in patients with severe craniocerebral injury.Methods A total of 96 patients with severe craniocerebral injury were divided into control group and observation group by different treatment schemes,with 48 cases in each group.The control group was treated with conventional craniotomy,and the observation group was treated with standard large bone flap decompression.The clinical effects of the two groups were compared.Results After treatment,the GCS,NIHSS and APACHE-Ⅱscores in the two groups improved,the levels of GFAP,S100βand MBP decreased,and the levels of CCP,CEO2 and SVO2 increased,and those in the observation group were better than the control group(P<0.05).The total incidence of postoperative complications in the observation group was lower than that in the control group,the good rate of recovery and GOS score were higher than those in the control group(P<0.05).Conclusion Standard large bone flap decompression in the treatment of patients with severe craniocerebral injury has significant effect,and it can effectively improve the neurological function,cerebral metabolic indexes and related serum index levels,reduce the incidence of postoperative complications and improve the prognosis of patients.
