基于数据挖掘分析中医药治疗冠心病合并失眠的用药规律

Data Mining Research on Treating Coronary Heart Disease with Insomnia by Traditional Chinese Medicine

目的分析冠心病合并失眠的临床用药规律。方法检索中国知网、维普、万方三大数据库,筛选冠心病合并失眠的中医药临床相关文献并进行整理,运用中医传承辅助平台(V2.5)进行药物频次分析、组方规律和新方分析。结果最终纳入方剂92首,涉及中药120味;常用单药有酸枣仁、丹参、甘草、川芎、茯苓,常用对药有丹参-酸枣仁、川芎-酸枣仁、甘草-酸枣仁、茯苓-酸枣仁、川芎-甘草,置信度较高的关联规则有远志→酸枣仁、知母→酸枣仁、川芎,甘草→酸枣仁,进而演化得到治疗冠心病合并失眠的核心药物8个、核心组合18个和新候选处方9首。结论通过对高频药物、组合、新候选处方的挖掘,发现中医药治疗冠心病合并失眠重视补虚药、活血化瘀药、清热药、利水渗湿药与安神药的配伍使用,标本兼顾,共同增强冠心病和失眠的临床治疗效果,本研究为冠心病合并失眠的临床遣方用药和新药研发提供理论依据。

Objective To analyze the medication rule of treating coronary heart disease(CHD) combined with insomnia.Methods Three databases including CNKI,VIP and Wanfang were searched to screen and sort out the clinical related literatures on TCM treatment of CHD with insomnia.The Chinese medicine inheritance platform was used to classify the efficacy of drugs,analyze the frequency of drugs,formula formation rule and new formula analysis,etc.Results A total of 92 prescriptions of TCM were collected involving 120 Chinese herbs;Among them,the commonly used single herbs were Semen Ziziphi Spinosae,Radix Salviae Miltiorrhizae,Radix Glycyrrhizae,Rhizoma Ligustici Chuanxiong,Poria;the commonly used paired herbs were Radix.Salviae Miltiorrhizae-Semen Ziziphi Spinosae,Rhizoma Ligustici ChuanxiongSemen Ziziphi Spinosae,Radix Glycyrrhizae-Zizyphus spinosus,Poria-Semen Ziziphi Spinosae,Rhizoma Ligustici Chuanxiong-Radix Glycyrrhizae;high association rules included Radix Polygalae→Semen Ziziphi Spinosae,Rhizoma Anemarrhenae→Semen Ziziphi Spinosae,Rhizoma Ligustici Chuanxiong-Radix Glycyrrhizae→Semen Ziziphi Spinosae.Then,we deduced 8 core herbs,18 core combinations and 9 new prescriptions for treating CHD with insomnia.Conclusion The combination of tonifying deficiency medicine,promoting blood circulation and removing blood stasis medicine,clearing heat medicine,diuresis and dampness medicine with tranquilizing medicine are important rules for clinical treatment of CHD and insomnia.This study provides theoretical basis for clinical prescription and new drug development especially for treatment CHD with insomnia.
