摘要
目的:界定不同层次的医养结合服务内容,探讨科学合理的医养结合服务提供模式。方法:通过大量文献,总结当前经验和存在的问题,以头脑风暴方法界定服务内容并且提出新的服务模式。结果:根据与居民需求相适宜,与机构供给能力相对称,经济性三个原则将医养结合服务内容以服务包的形式划分为:居家养老服务包、社区照护服务包、机构养老服务包、医院医疗护理服务包、增量自选服务包;提出了"三+三"的服务模式。结论:新模式的推行需要突破各种限制性因素。目前面临的主要困境有制度环境不完善,人才短缺,信息化建设水平较低。
Objective To define the content of integrated elderly care and health service at different levels,and explore the scientific and reasonable model for elderly care and health service delivery.Methods System literature review was conduct to summarize the experiences and existing problems in elderly care and health service delivery system.Brainstorming was conduct to propose a new service delivery model.Results According to three principles of economy,orientation towards residents’needs and suitability for the supply capacity of institutions,the integrated elderly care and health services were divided into five sub-packages,including home-based care service package,community care service package,institutional care service package,medical care service package and optional service package.And the service mode of"three+three"was constructed.Conclusion The promotion of the new service mode needs to break through various restrictive factors,which is facing with a series of practical difficulties,such as imperfect system environment,shortage of talents and low level of information construction.
