混合痔患者痔动脉结扎悬吊术治疗效果的影响因素

Influencing Factors of Hemorrhoid Artery Ligation and Suspension in Patients with Mixed Hemorrhoids

目的探讨混合痔患者痔动脉结扎悬吊术治疗效果的影响因素。方法选取2018年1月至2020年12月于三门峡市第三人民医院接受痔动脉结扎悬吊术的172例混合痔患者为研究对象,观察患者治疗效果,并收集患者临床资料,分析可能影响混合痔患者痔动脉结扎悬吊术治疗效果的相关因素。结果172例混合痔患者痔动脉结扎悬吊术治疗有效134例(77.91%),无效38例(22.09%)。无效组便秘、病灶表现、2型糖尿病、病灶直径与有效组比较,差异有统计学意义(P<0.05)。多元logistic回归分析结果显示,便秘、环状脱出或病灶边界不清、2型糖尿病、病灶直径较长是混合痔患者痔动脉结扎悬吊术治疗无效的影响因素(OR>1,P<0.05)。结论混合痔患者痔动脉结扎悬吊术总体治疗效果较好,但仍有部分患者治疗无效,便秘、环状脱出或病灶边界不清、2型糖尿病、病灶直径较长是混合痔患者痔动脉结扎悬吊术治疗无效的影响因素。

Objective To explore the influencing factors of hemorrhoid artery ligation and suspension in patients with mixed hemorrhoids.Methods A total of 172 patients with mixed hemorrhoids who underwent hemorrhoid artery ligation and suspension in Sanmenxia Third People’s Hospital from January 2018 to December 2020 were selected as the research subjects.The therapeutic effect of patients was observed,and the clinical data of patients were collected.The related factors that might affect the therapeutic effect of hemorrhoid artery ligation and suspension in patients with mixed hemorrhoids were analyzed.Results In 172 patients with mixed hemorrhoids after ligation and suspension of hemorrhoid artery,134 cases(77.91%)were effective and 38 cases(22.09%)were ineffective.There were differences in the constipation,lesion performance,type 2 diabetes and lesion diameter between the ineffective group and the effective group(P<0.05).Multivariate logistic regression analysis showed that constipation,annular prolapse or borderline lesion,type 2 diabetes,and longer lesion diameter were the influencing factors of ineffective treatment of hemorrhoidal artery ligation and suspension for mixed hemorrhoids patients(OR>1,P<0.05).Conclusion Hemorrhoid artery ligation and suspension has good effect in the treatment of patients with mixed hemorrhoids,but some patients are still ineffective.Constipation,annular prolapse or borderline lesion,type 2 diabetes mellitus,and longer lesion diameter are the influencing factors of ineffective treatment of hemorrhoidal artery ligation and suspension in patients with mixed hemorrhoids.
