血清4种肿瘤标志物水平与肝硬化患者Child-Pugh分级的相关性分析

Correlation between Serum Levels of Four Tumor Markers and Child Pugh Grade in Patients with Liver Cirrhosis

目的分析血清4种肿瘤标志物水平与肝硬化(LC)Child-Pugh分级的关系。方法选取2019年3月至2020年3月郏县人民医院收治的112例LC患者和90例同期健康体检者作为研究对象。检测其血清肿瘤标志物水平[糖类抗原199(CA199)、糖类抗原125(CA125)、癌胚抗原(CEA)、甲胎蛋白(AFP)],评估Child-Pugh分级。分析LC患者血清肿瘤标志物水平与Child-Pugh分级的相关性及血清肿瘤标志物水平单独与联合检测对LC的诊断价值。结果LC患者血清CA199、CA125、CEA、AFP水平高于健康体检者(P<0.05)。随着Child-Pugh分级升高,血清CA199、CA125、CEA、AFP水逐渐升高(P<0.05)。经Spearman相关性分析,血清CA199、CA125、CEA、AFP水平与Child-Pugh分级呈正相关(P<0.05)。经受试者工作特征(ROC)曲线分析,联合检测诊断LC患者的曲线下面积(AUC)(0.848)高于CA199(0.725)、CA125(0.701)、CEA(0.736)、AFP(0.741)单独检测(P<0.05)。结论LC患者血清肿瘤标志物水平明显异常升高,且与Child-Pugh分级具有关联性,联合检测对病情的诊断价值较单独检测高。

Objective To analyze the correlation between the levels of four tumor markers in serum and Child-Pugh grade in patients with liver cirrhosis(LC).Methods A total of 112 patients with LC and 90 healthy subjects in Jiaxian people’s Hospital from March 2019 to March 2020 were selected as the research objects.The levels of serum tumor markers[carbohydrate antigen 199(CA199),carbohydrate antigen 125(CA125),carcinoembryonic antigen(CEA)and alpha fetoprotein(AFP)]were measured to evaluate the Child-Pugh grade.The correlations between serum tumor marker level and Child-Pugh grade in patients with LC were analyzed,and the diagnostic values of serum tumor marker level alone and combined detection in LC were analyzed.Results The levels of serum CA199,CA125,CEA and AFP in patients with LC were higher than those in healthy subjects(P<0.05).With the increase of Child-Pugh grade,the levels of serum CA199,CA125,CEA,AFP were increased gradually(P<0.05).Spearman correlation analysis showed that the levels of serum CA199,CA125,CEA and AFP were positively correlated with Child-Pugh grade(P<0.05).According to the analysis of receiver operating characteristic(ROC)curve,the area under the curve(AUC)(0.848)of LC patients diagnosed by combined detection was higher than that detected by CA199(0.725),CA125(0.701),CEA(0.736)and AFP(0.741)alone(P<0.05).Conclusion The levels of serum tumor markers in patients with LC is significantly abnormally elevated,which is related to Child-Pugh grade.The diagnostic value of combined examination is higher than that of single diagnosis.
