球形肺炎、肺结核球与周围型肺癌的CT诊断及鉴别

CT Diagnosis and Differential Diagnosis of Spherical Pneumonia,Tuberculoma and Peripheral Lung Cancer

目的探究球形肺炎、肺结核球与周围型肺癌的CT表现。方法选取2020年4月至2021年4月郑州市第十五人民医院收集的85例肺部疾病患者的临床资料作为研究对象,包括31例球形肺炎、28例周围型肺癌和26例肺结核球,分析三者的CT影像学表现。结果球形肺炎、肺结核球、周围型肺癌在肺部的分布位置比较,差异无统计学意义(P>0.05)。球形肺炎、肺结核球与周围型肺癌病灶大小、病灶边缘、病灶密度、肿大淋巴结、空气支气管征、CT强化特点、分叶征、胸膜凹陷征等比较,差异有统计学意义(P<0.05)。结论CT平扫和增强扫描中的球形肺炎、肺结核球与周围型肺癌病灶分布位置相当,应通过综合分析对三者进项鉴别。

Objective To explore the differences in CT manifestations of spherical pneumonia,pulmonary tuberculosis and peripheral lung cancer.Methods The clinical data of 85 patients with lung diseases collected in Zhengzhou 15th people’s Hospital from April 2020 to April 2021 were selected as the research object,including 31 cases of spherical pneumonia,there were 28 cases of peripheral lung cancer and 26 cases of pulmonary tuberculoma.The CT imaging findings of the three were analyzed.Results There were no differences in the distribution of globular pneumonia,tuberculoma and peripheral lung cancer(P>0.05).There were differences in lesion size,lesion edge,lesion density,enlarged lymph nodes,air bronchial sign,CT enhancement characteristics,lobulation sign and pleural depression sign between spherical pneumonia and pulmonary tuberculoma and peripheral lung cancer(P<0.05).Conclusion The distribution of spherical pneumonia and tuberculoma in plain and enhanced CT scan is similar to that of peripheral lung cancer.They should be differentiated by comprehensive analysis of the results.
