自制新型体位垫八字形摆放在超低位直肠癌患者手术室体位护理中的应用效果

Application Effect of Self-Made New Type of Posture Cushion in Eight-Shaped Arrangement in Posture Nursing of Patients With Ultra-Low Rectal Cancer in Operating Room

目的分析自制新型体位垫八字形摆放在超低位直肠癌患者手术室体位护理中的应用效果。方法选取2020年3月至2021年4月南阳市中心医院收治的85例超低位直肠癌患者作为研究对象,患者均接受腹腔镜直肠癌保肛手术治疗,将术中接受改良截石位且无需特殊干预的42例作为对照组,将术中接受改良截石位且自制新型体位垫八字形摆放的43例作为观察组。比较两组不同时间点[体位改变前(T_(0))、体位改变后5 min(T_(1))、术毕恢复正常体位前(T_(2))、恢复正常体位后5 min(T 3)]血压、中心静脉压(CVP)、眼压、舒适度。结果观察组T_(1)、T_(2)时的收缩压、舒张压较对照组低,T 3时的收缩压、舒张压较对照组高(P<0.05);观察组T_(1)、T_(2)、T 3时CVP均较对照组低(P<0.05);观察组T_(1)、T_(2)、T 3时眼压均较对照组低(P<0.05);观察组舒适度较对照组高(P<0.05)。结论将自制新型体位垫八字形摆放应用于超低位直肠癌患者手术室体位护理中,能稳定血压、CVP及眼压水平,提高舒适度。

Objective To analyze the application effect of self-made new posture mats placed in eight-shape in the posture nursing of patients with ultra-low rectal cancer in the operating room.Methods A total of 85 patients with ultra-low rectal cancer in Nanyang Central Hospital from March 2020 to April 2021 were selected as the research subjects.All patients received laparoscopic rectal cancer anus-preserving surgery.At the same time,42 patients who received a modified lithotomy position during the operation and did not require special intervention as control group,43 patients who received an improved lithotomy position and self-made a new type of posture mat were placed in a figure eight shape during the operation were used as observation group.Compare the two groups at different time points[before the body position change(T_(0)),5 minutes after the body position change(T_(1)),before the normal position after the operation(T_(2)),and 5 minutes after the normal position(T 3)]blood pressure,central venous pressure(CVP)),intraocular pressure and comfort.Results The systolic blood pressure and diastolic blood pressure of the observation group at T_(1)and T_(2)were lower than those of the control group,and the systolic blood pressure and diastolic blood pressure at T 3 were higher than those of the control group(P<0.05).The CVP at T_(1),T_(2),and T 3 of the observation group was lower than that of the control group(P<0.05).The intraocular pressure at T_(1),T_(2),and T 3 of the observation group was lower than that of the control group(P<0.05).The comfort level of the observation group was higher than that of the control group(P<0.05).Conclusion The self-made new type of posture mat is used in the posture care of patients with ultra-low rectal cancer in the operating room,which can stabilize blood pressure,CVP,and intraocular pressure,and improve comfort.
