闭合复位经皮内固定治疗肱骨外科颈骨折临床探究

Closed Reduction and Percutaneous Internal Fixation for the Treatment of Fractures of Surgical Neck of Humerus

目的研究闭合复位经皮克氏针内固定疗法在肱骨外科颈骨折患者治疗中的应用价值。方法选取该院2017年8月—2020年8月肱骨外科接收的120例颈骨折患者作为研究对象,按照治疗方法对其进行分组,将采取手法复位治疗的50例患者分为手法复位组,将70例采取闭合复位经皮克氏针内固定方法进行复位治疗的患者分为内固定组。比较两组治疗效果。结果内固定组治疗有效率明显高于手法复位组(95.70%>72.00%),差异有统计学意义(χ^(2)=13.489,P<0.05);内固定组手术时间、住院和骨折愈合时间与手法复位组相比明显较短,与手法复位组相比术中出血量明显较少,差异有统计学意义(P<0.05);内固定组并发症总发生率明显低于手法复位组(4.29%<32.00%),差异有统计学意义(P<0.05)。结论与手法固定疗法相比,在肱骨外科颈骨折的治疗中采用复位经皮克氏针内固定疗法临床治疗有效率显著提高,应用价值较高。

Objective To study the application value of closed reduction and percutaneous Kirschner wire internal fixation in the treatment of patients with humeral surgical neck fractures.Methods A total of 120 patients with neck fractures received by the department of humeral surgery from August 2017 to August 2020 were selected as the research objects.The patients were divided into groups according to the treatment method.50 patients who underwent manual reduction treatment were divided into manual reduction group.70 patients who were treated with closed reduction and percutaneous kirschner wire internal fixation were divided into the internal fixation group.Compared the treatment effects of the two groups.Results The treatment efficiency of the internal fixation group was significantly higher than that of the manual reduction group(95.70%>72.00%),and the difference was statistically significant(χ^(2)=13.489,P<0.05);the operation time,hospitalization and fracture healing time and manual reduction in the internal fixation group compared with the manual reduction group,the intraoperative blood loss was significantly less and the difference was statistically significant(P<0.05);the total incidence of complications in the internal fixation group was significantly lower than that in the manual reduction group(4.29%<32.00%),the difference was statistically significant(P<0.05).Conclusion Compared with manual fixation therapy,reduction and percutaneous Kirschner wire internal fixation in the treatment of humeral surgical neck fractures has a significantly higher clinical treatment efficiency and higher application value.
