输尿管软镜联合微创经皮肾镜治疗复杂性肾结石疗效及并发症分析

Analysis of the Efficacy and Complications of Flexible Ureteroscopy Combined with Minimally Invasive Percutaneous Nephroscope in the Treatment of Complex Renal Calculus

目的分析输尿管软镜联合微创经皮肾镜治疗复杂性肾结石疗效及并发症情况。方法选取2019年1月—2020年1月该院收治的复杂性肾结石78例患者临床资料,按随机数表分为两组。对照组39例使用微创经皮肾镜治疗,研究组39例使用输尿管软镜联合微创经皮肾镜治疗,对两组疗效及并发症进行比较。结果研究组患者手术时间(52.38±10.19)min和住院时间(8.12±1.06)d比对照组短,差异有统计学意义(t=8.626、19.645,P<0.05);治疗后,研究组Scr、BUN与Upro等指标均优于对照组,差异有统计学意义(t=4.399、4.230、3.478,P<0.05);研究组尿瘘、发热、术后出血等并发症率5.12%低于对照组20.51%,差异有统计学意义(χ^(2)=4.129,P<0.05);研究组生活质量(89.11±8.24)分高于对照组,差异有统计学意义(t=7.947,P<0.05)。结论临床使用输尿管软镜联合微创经皮肾镜治疗复杂性肾结石患者,能减短患者手术时间和住院时间,减少术后并发症发生,还能提高其生活质量,临床上值得推广使用。

Objective To analyze the efficacy and complications of flexible ureteroscopy combined with minimally invasive percutaneous nephroscope in the treatment of complex kidney stones.Methods The clinical data of 78 patients with complex renal calculus admitted to the hospital from January 2019 to January 2020 were selected and divided into two groups according to the random number table.The control group 39 cases were treated with minimally invasive percutaneous nephroscope,and the study group 39 cases using flexible ureteroscopy combined with minimally invasive percutaneous nephroscope treatment,the efficacy and complications of the two groups were compared.Results The operation time(52.38±10.19)min and the hospitalization time(8.12±1.06)d in the study group were shorter than those in the control group,and the difference were statistically significant(t=8.626,19.645,P<0.05).After treatment,the study group Scr,BUN and Upro and other indicators were better than those of the control group,and the difference was statistically significant(t=4.399,4.230,3.478,P<0.05);the complication rate of urinary fistula,fever,postoperative bleeding in the study group was 5.12%lower than that of the control group 20.51%,the difference was statistically significant(χ^(2)=4.129,P<0.05).The quality of life(89.11±8.24)points of the study group was higher than that of the control group,and the difference was statistically significant(t=7.947,P<0.05).Conclusion The clinical use of flexible ureteroscopy combined with minimally invasive percutaneous nephrolithiasis for the treatment of patients with complex renal calculus can shorten the operation time and hospital stay of patients,reduce postoperative complications,and improve their quality of life.It was clinically worthy of promotion.
