乳腺癌合并糖尿病患者围术期护理效果及患者满意的比例

Perioperative Nursing Effect and Satisfactory Proportion of Patients with Breast Cancer and Diabetes

目的探讨乳腺癌合并糖尿病患者围术期护理效果及患者满意的比例。方法将五华县人民医院普外科2019年1月-2020年1月80例乳腺癌合并糖尿病患者,随机分组,普通干预组给予常规护理,全方位干预组实施围术期全方位护理。比较两组护理前后HbA1c、不同状态下血糖监测值和W.K.Zung编制的负面情绪评分表分数、护理满意的比例、手术全程的时间以及住院总天数、切口感染和低血糖发生率。结果护理后全方位干预组HbA1c、不同状态下血糖监测值、W.K.Zung编制的负面情绪评分表分数显著低于普通干预组,差异有统计学意义(P<0.05)。全方位干预组的护理满意的比例比普通干预组高,差异有统计学意义(P<0.05)。全方位干预组手术全程的时间及住院总天数均短于普通干预组,差异有统计学意义(P<0.05)。普通干预组有4例切口感染和4例低血糖,发生率为20.00%,全方位干预组有1例切口感染和1例低血糖,发生率为5.00%,差异有统计学意义(P<0.05)。结论乳腺癌合并糖尿病患者实施围术期全方位护理效果确切,有利于血糖控制,并减轻患者的心理压力,缩短手术和住院总天数,减少切口感染和低血糖的发生,提高患者对于护理工作的满意的比例。

Objective To explore the perioperative nursing effect of patients with breast cancer and diabetes mellitus and the proportion of patients’satisfaction.Methods From January 2019 to January 2020,80 patients with breast cancer and diabetes in the department of general surgery of Wuhua County People’s Hospital were randomly divided into groups.The general intervention group was given routine care,and the comprehensive intervention group was given perioperative care.The HbA1 c before and after nursing,the blood glucose monitoring value under different conditions and the scores of the negative emotion scale compiled by W.K.Zung,the proportion of nursing satisfaction,the time of the whole operation,the total days of hospitalization,the incidence of incision infection and hypoglycemia were compared between the two groups.Results The scores of HbA1 c,blood glucose monitoring values under different conditions,and negative emotion scores compiled by W.K.Zung in the comprehensive intervention group after nursing were significantly lower than those in the ordinary intervention group,the difference was statistically significant(P<0.05).The percentage of nursing satisfaction in the comprehensive intervention group was higher than that in the general intervention group,the differences was statistically significant(P<0.05).The total operation time and the total number of days of hospitalization in the omnidirectional intervention group were shorter than those in the general intervention group,the differences was statistically significant(P<0.05).In the general intervention group,there were 4 cases of incision infection and 4 cases of hypoglycemia,with an incidence of 20.00%,and the comprehensive intervention group had 1 case of incision infection and 1 case of hypoglycemia,with an incidence of 5.00%,the differences was statistically significant(P<0.05).Conclusion The effect of perioperative comprehensive care for breast cancer patients with diabetes mellitus is definite,which is beneficial to blood glucose control,reduces the patient’s psychological pressure,shortens the total days of surgery and hospitalization,reduces the incidence of incision infection and hypoglycemia,and improves the patient’s satisfactory ratio to the nursing work.
