团队协同管理方案在2型糖尿病患者护理中的应用

Application of Team Management Plan in the Nursing of Type 2 Diabetes Patients

目的探究团队协同管理方案在2型糖尿病(Type 2 diabetes,T2DM)患者护理中的应用效果。方法将该院2019年1-7月收治的92例T2DM患者按照信封法随机分成A组45例(观察组)与B组47例(对照组)。B组进行常规护理,A组则在B组基础上接受团队协同管理方案。观察两组生活质量,并比较干预前、后两组空腹血糖、餐后2 h血糖、心理状况(HAMD、HAMA评分)及自护能力(ESCA评分)。结果两组干预后空腹血糖及餐后2 h血糖均较干预前下降,且A组干预后各指标低于B组,差异有统计学意义(P<0.05);两组干预后HAMD、HAMA评分较干预前均下降,且A组干预后各评分低于B组,差异有统计学意义(P<0.05);A组干预后自护责任感、自护概念、自护能力及健康知识水平评分较干预前上升,且A组各评分高于B组,差异有统计学意义(P<0.05);A组干预后生理机能、精力、精神健康、生理职能评分均高于B组,差异有统计学意义(P<0.05)。结论对2型糖尿病患者采取团队协同管理方案有助于帮助患者控制血糖,加强其自护能力,有效提高其生活质量。

Objective To explore the application effect of team collaborative management program in the care of Type 2 diabetes(T2 DM)patients.Methods Ninety-two patients with T2 DM admitted to the hospital from January to July 2019 were randomly divided into 45 cases in group A(observation group)and 47 cases in group B(control group)according to the envelope method.Group B was given routine nursing care,and Group A received a collaborative team management plan on the basis of Group B.The quality of life of the two groups was observed,and the fasting blood glucose,2 h postprandial blood glucose,psychological status(HAMD,HAMA score)and self-care ability(ESCA score)were compared between the two groups before and after the intervention.Results The fasting blood glucose and 2 h postprandial blood glucose after the intervention of the two groups were lower than before the intervention,and the indicators of group A were lower than those of group B after intervention,the differences were statistically significant(P<0.05);the HAMD and HAMA scores of the two groups decreased after intervention compared with before intervention,and after the intervention,the scores of group A were lower than those of group B,the differences were statistically significant(P<0.05);the scores of self-care responsibility,selfcare concept,self-care ability and health knowledge level after intervention of group A were higher than those before intervention,and the scores of group A were higher than those of group B,the differences were statistically significant(P<0.05);The scores of physiological function,energy,mental health and physiological function,in group A were higher than those in group B after intervention,the differences were statistically significant(P<0.05).Conclusion Adopting a team management plan for patients with type 2 diabetes can help patients control blood glucose,strengthen their self-care ability,and effectively improve their quality of life.
