摘要
目的探讨利用三维能量多普勒超声在鉴别子宫内膜恶性和子宫内膜良性病变之间的诊断价值。方法收集2019年1月至2020年12月在我院就诊行三维多普勒超声检查的疑似子宫内膜病变患者302例,使用虚拟计算机辅助分析软件(VOCAL)测量子宫内膜厚度(ET)、子宫内膜容积(EV)、血管形成指数(VI)、血流指数(FI)和血管形成-血流指数(VFI),以子宫内膜病理检查结果为诊断金标准,计算三维能量多普勒超声诊断子宫内膜良恶性病变的灵敏度和特异度。结果 (1)三维能量多普勒超声诊断恶性子宫内膜病变的敏感度、特异度、阳性预测值、阴性预测值分别为93.48%、97.27%、86%和98.81%;(2)恶性子宫内膜病变患者ET、EV及血流参数VI、FI、VFI均明显高于良性子宫内膜病变者,差异有统计学意义(P<0.05)。结论三维能量多普勒超声对子宫内膜良恶性病变的诊断准确率较高,为临床医生正确诊断疾病提供有力依据。
Objective To investigate the diagnostic value of 3-D Doppler ultrasonography in differentiating endometrial malignant lesion from benign lesion.Methods A total of 302 patients with suspected endometrial lesions who received 3-D Doppler ultrasonography in our hospital from January 2019 to December 2020 were collected,the results of endometrial pathology used as the gold standard for diagnosis.VOCAL software was used to calculate the endometrial thickness(ET),intimal volume (EV),vascularization index (VI),blood flow index (FI) and vascularization-blood flow index (VFI),calculating the sensitivity and specificity of 3D energy Doppler ultrasonography in the diagnosis of benign and malignant endometrial lesions.Results (1)The sensitivity,specificity,positive predictive value and negative predictive value of 3D ultrasound in the diagnosis of endometrial malignant lesions were 93.48%,97.27%,86% and 98.81%,respectively.(2) ET,EV and blood flow parameters (VI,FI,VFI) in patients with malignant endometrium lesions were significantly higher than those in patients with benign endometrium lesions,and the differences were statistically significant (P<0.05).Conclusion The accuracy of 3-D energy Doppler ultrasonography in the diagnosis of benign and malignant endometrial lesions is high,and it provides a strong basis for clinicians to diagnose diseases correctly.
