鼻腔冲洗与丙酸氟替卡松在鼻窦炎鼻内镜手术后的应用价值

Application Value of Nasal Cavity Irrigation and Fluticasone Propionate After Endoscopic Sinus Surgery

目的探究在鼻窦炎鼻内镜手术后运用鼻腔冲洗与丙酸氟替卡松联合治疗的临床价值。方法选取130例在2018年1月-2019年12月该院收治的鼻窦炎鼻内镜手术患者,随机分为观察组(鼻腔冲洗与丙酸氟替卡松联合治疗)和对照组(单纯鼻腔冲洗治疗)各65例。比较两组临床效果。结果与对照组相比,观察组治疗总有效率(96.92%)较高,差异有统计学意义(χ^(2)=10.263,P<0.05);观察组术腔鼻黏膜上皮化时间(6.25±1.26)d、术腔水肿消退时间(4.58±1.57)d、疼痛消失时间(3.21±0.27)d、鼻塞消失时间(2.92±0.14)d均较短,差异有统计学意义(t=11.695、9.289、36.033、47.376,P<0.05);治疗后,观察组血清白细胞介素-6(51.66±3.41)pg/mL、肿瘤坏死因子(96.32±10.55)ng/L及C反应蛋白(3.42±0.21)mg/L水平均较低,差异有统计学意义(t=24.100、30.708、15.056,P<0.05),且观察组生存质量各指标水平均较高,差异有统计学意义(t=4.642、12.371、6.722、6.592、6.904、7.406,P<0.05)。结论在鼻窦炎鼻内镜手术后运用鼻腔冲洗与丙酸氟替卡松联合治疗,能够提高治疗效果,缩短患者的症状恢复时间,改善患者的炎症反应,同时还能够提升患者的生活质量。

Objective To explore the clinical value of combined treatment of nasal cavity irrigation and fluticasone propionate after endoscopic sinus surgery.Methods 130 patients with sinusitis and endoscopic sinus surgery admitted to the hospital from January 2018 to December 2019 were selected and randomly divided into observation group(nasal cavity irrigation combined with fluticasone propionate treatment)and control group(nasal cavity irrigation treatment alone)65 people each.Compare the clinical effects of the two groups.Results Compared with the control group,the observation group has a higher total effective rate of treatment(96.92%),and the difference was statistically significant(χ^(2)=10.263,P<0.05).the observation group’s nasal mucosal epithelialization time(6.25±1.26)d,surgical cavity edema subsided time(4.58±1.57)d,pain disappeared time(3.21±0.27)d,nasal congestion disappeared time(2.92±0.14)d were all shorter,the difference was statistically significant(t=11.695,9.289,36.033,47.376,P<0.05).After treatment,serum interleukin-6(51.66±3.41)pg/mL,tumor necrosis factor(96.32±10.55)ng/L and C-reactive protein(3.42±0.21)mg/L levels in the observation group all indicators were low,and the difference was statistically significant(t=24.100,30.708,15.056,P<0.05),and the quality of life indicators in the observation group were relatively high,and the difference was statistically significant(t=4.642,12.371,6.722,6.592,6.904,7.406,P<0.05).Conclusion The combined treatment of nasal cavity irrigation and fluticasone propionate after sinusitis endoscopic sinus surgery can improve the treatment effect,shorten the patient’s symptom recovery time,improve the patient’s inflammatory response,and improve the patient’s quality of life.
