Nursing Care of External Fixator Combined with Limited Internal Fixation to Treat Hand Fracture

目的分析外固定支架结合有限内固定治疗手部骨折的护理方法及效果,探讨有效的护理模式,以提高临床疗效,促进患者尽早康复。方法方便选取该院骨科2019年6月—2020年6月收治的94例手部骨折患者,所有患者均行外固定支架结合有限内固定治疗,根据治疗期间的不同护理方法将其纳入A组与B组各47例,A组实施常规护理,B组实施综合护理干预,对比两组患者的心理状态(SAS评分、SDS评分)、疼痛程度(VAS评分)、日常生活自理能力(ADL评分)、住院时间与骨折愈合时间、康复效果以及护理满意度。结果B组护理后的SAS、SDS评分(50.81±3.36)分、(47.60±3.45)分均低于A组(59.57±3.74)分、(56.49±3.82)分,差异有统计学意义(t=11.945、11.840,P<0.05);B组护理后的VAS评分(1.79±0.67)分低于A组(3.08±0.90)分,差异有统计学意义(t=7.882,P<0.05);B组护理后的ADL评分(85.53±7.17)分高于A组(74.14±9.58)分,差异有统计学意义(t=6.526,P<0.05);B组的住院时间与骨折愈合时间为(11.05±1.45)d、(12.44±1.62)周均短于A组(17.25±1.93)d、(19.71±1.96)周,差异有统计学意义(t=17.608、19.600,P<0.05);B组康复效果的优良率(78.72%)高于A组(59.57%),差异有统计学意义(χ^(2)=4.039,P=0.044);B组的护理满意度(100.00%)高于A组(87.23%),差异有统计学意义(χ^(2)=4.451,P=0.035)。结论外固定支架结合有限内固定治疗手部骨折的效果确切,治疗期间对患者实施综合护理干预,能够明显改善患者的不良情绪,减轻疼痛,提高日常生活自理能力,且可缩短住院时间与骨折愈合时间,有助于患者尽早康复,护理满意度高,值得推行。

Objective To analyze the nursing methods and effects of external fixation combined with limited internal fixation in the treatment of hand fractures,and to explore effective nursing models to improve clinical efficacy and promote early recovery of patients.Methods Conveniently select 94 hand fracture patients admitted to the Department of Orthopedics of the hospital from June 2019 to June 2020 were selected.All patients received external fixation combined with limited internal fixation.They were included in group A and group B according to different nursing methods during the treatment period.There were 47 cases in each group.Group A was provided with routine nursing care and group B was provided with comprehensive nursing intervention.The psychological status(SAS score,SDS score),pain degree(VAS score),self-care ability(ADL score),and hospitalization time and fracture healing time,rehabilitation effect and nursing satisfaction of the two groups were compared.Results The SAS and SDS scores(50.81±3.36)points and(47.60±3.45)points of group B after nursing were lower than those of group A(59.57±3.74)points and(56.49±3.82)points,the difference was statistically significant(t=11.945,11.840,P<0.05);The VAS score(1.79±0.67)points of group B after nursing was lower than that of group A(3.08±0.90)points,the difference was statistically significant(t=7.882,P<0.05);group B of the ADL score(85.53±7.17)after nursing was higher than that of group A(74.14±9.58)points,the difference was statistically significant(t=6.526,P<0.05);the hospital stay and fracture healing time of group B(11.05±1.45)d and(12.44±1.62)weeks were shorter than group A(17.25±1.93)d and(19.71±1.96)weeks,the difference was statistically significant(t=17.608,19.600,P<0.05);the excellent and good rate of the rehabilitation effect of group B was(78.72%)was higher than that of group A(59.57%),the difference was statistically significant(χ^(2)=4.039,P=0.044);the nursing satisfaction of group B(100.00%)was higher than that of group A(87.23%),the difference was statistically significant(χ^(2)=4.451,P=0.035).Conclusion The effect of external fixation combined with limited internal fixation in the treatment of hand fractures is definite.Implementing comprehensive nursing intervention during the treatment period can significantly improve the patient’s unhealthy mood,relieve pain,improve daily life self-care ability,and shorten hospital stay and fracture healing time,which helps patients recover as soon as possible,and the nursing satisfaction is high,which is worthy of implementation.
