丁苯酞联合复方丹参注射液治疗急性缺血性脑卒中的临床效果评价

Clinical evaluation of butylphthalide combined with compound Danshen injection in the treatment of acute ischemic stroke

目的:研究丁苯酞注射液联合复方丹参注射液治疗急性缺血性脑卒中的临床效果。方法:采用前瞻性研究方法,按随机数字表法将2019年9月–2020年9月我院收治的208例急性缺血性脑卒中患者进行分组,分为对照组和观察组,每组各104例。对照组使用丁苯酞注射液,观察组在对照组基础上加用复方丹参注射液,疗程14 d,比较两组治疗后的临床疗效、日常生活能力(ADL)评分、生化指标、血液流变学等情况。结果:两组治疗后,观察组的总有效率为90.38%,高于对照组(73.08%),P<0.05;观察组ADL评分高于对照组,P<0.05。治疗后两组内皮素-1、C反应蛋白、同型半胱氨酸水平均下降,但观察组上述指标均低于对照组,P<0.05;观察组血浆黏度、纤维蛋白原、全血黏度高切、全血黏度低切均低于对照组,P<0.05。结论:丁苯酞联合复方丹参注射液治疗急性缺血性脑卒中的效果明显优于丁苯肽单药治疗,值得临床借鉴。

Objective:To study the clinical effect of butylphthalide combined with compound Danshen injection in the treatment of acute ischemic stroke.Methods:208 patients with acute ischemic stroke who were admitted into our hospital from September 2019 to September 2020 were prospectively divided into the control group and the observation group,with 104 cases in each group.The control group was given butylphthalide injection for 14 days,and the observation group was given compound Danshen injection combined with butylphthalide injection for 14 days.And then the clinical effect,the score of the activities of daily living(ADL),index of biochemistry and hemorheology of the two groups were compared.Results:After treatment,the total effective rate in the observation group(90.38%)was significantly higher than that in the control group(73.08%),P<0.05;The ADL score in observation group was also significantly higher than that in control group,P<0.05.The serum level of endothelin-1,C-reactive protein(CRP)and homocysteine were all decreased after treatment and those in the observation group were significantly lower than those in the control group,P<0.05.The plasma viscosity,fibrinogen,high whole blood viscosity,and low whole blood viscosity of observation group were significantly lower than those of the control group,P<0.05.Conclusion:The effect of butylphthalide combined with compound Danshen injection in the treatment of acute ischemic stroke is significantly better than that of butylphthalide monotherapy,which is worthy of clinical reference.
