维持性血液透析患者透析中血压变异水平及其影响因素分析 被引量:1

Analysis of blood pressure variation and its influencing factors in patients undergoing maintenance hemodialysis

摘要 目的探讨维持性血液透析(MHD)患者透析中血压变异性的影响因素。方法采用回顾性病例对照研究设计,选取2020年1月至6月在广东省人民医院、深圳市中医院血液透析中心接受MHD患者278例,男152例(54.7%),女126例(45.3%),中位年龄57岁,中位透析龄67个月,糖尿病患者89例(32.0%)。根据计算收缩压血压变异性(BPV)中位数,将研究对象分为高BPV组(BPV>10.05)和低BPV组(BPV≤10.05)。采集其2020年6月每次血液透析治疗透析前、透析中每小时及透析后血压数据,计算透析中血压变异性。结果高BPV组和低BPV组患者的年龄(Z=3.402,P=0.001)、糖尿病(χ^(2)=10.629,P=0.001)、超滤率(Z=-2.112,P=0.036)、血清白蛋白(t=2.272,P=0.024)、β2微球蛋白(Z=-2.166,P=0.030)比较差异均有统计学意义(均P<0.05)。多因素logistic回归分析结果显示,年龄(OR=1.027,95%CI:0.018~1.045,P=0.005)、糖尿病(OR=1.876,95%CI:11.076~3.272,P=0.027)、超滤率(OR=2.571,95%CI:1.471~4.491,P=0.001)是透析中高BPV的独立危险因素。结论超滤率是影响MHD患者透析中血压变异的主要危险因素,对于年龄越大、有糖尿病的患者尤其应重点关注。
