健心颗粒治疗老年气虚血瘀型慢性心力衰竭临床观察

Efficacy observation of Jianxin granule on senile chronic heart failure with syndrome of qi deficiency and blood stasis

目的:观察健心颗粒治疗老年气虚血瘀型慢性心力衰竭患者的临床效果。方法:将符合纳入标准的老年气虚血瘀型慢性心衰患者86例,随机分为两组。对照组予常规西药治疗,治疗组在对照组基础上加服健心颗粒。两组疗程均为8周,疗程结束后根据中医证候积分评价中医疗效,行6 min步行试验(6MWT),检测左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)和N末端脑钠肽前体(NT-proBNP)评估心功能变化。结果:治疗组显效25例,有效11例,无效6例,总有效率为85.71%;对照组显效20例,有效6例,无效14例,总有效率为65.00%,两组疗效比较,差异有统计学意义(P﹤0.05);治疗后,两组6MWT、LVEF、LVEDD、NT-proBNP均较治疗前有所改善(P﹤0.05),且治疗组优于对照组(P﹤0.05)。结论:健心颗粒可以提高老年气虚血瘀型慢性心力衰竭患者的中医证候疗效,提高运动耐力,改善心功能。

Objective:To observe the clinical effect of Jianxin granule on senile chronic heart failure with syndrome of qi deficiency and blood stasis.Methods:86 cases of senile patients with chronic heart failure of qi deficiency and blood stasis type,who met the inclusion criteria,were randomly divided into two groups.The control group was treated with conventional western medicine;while the treatment group was treated plus with Jianxin granule on the basis of control group.For both groups,the course of treatment was 8 weeks.After the end of course of treatment,TCM curative effect was evaluated by TCM syndrome score;6 minutes walk test(6MWT)was performed;left ventricular ejection fraction(LVEF),left ventricular end diastolic diameter(LVEDD)and N-terminal pro-brain natriuretic peptide(NT-proBNP)were detected in order to evaluate the change of cardiac function.Results:In the treatment group,25 cases were markedly effective,11 cases were effective,6 cases were ineffective,and the total effective rate was 85.71%;while in the control group,20 cases were markedly effective,6 cases were effective,14 cases were ineffective,and the total effective rate was 65.00%;compared between two groups,the difference was statistically significant(P﹤0.05).After treatment in both groups,6MWT,LVEF,LVEDD and NT-proBNP were improved compared with before treatment(P﹤0.05),and the treatment group was better than control group(P﹤0.05).Conclusion:Jianxin granule can improve the curative effect of TCM syndrome of patients with senile chronic heart failure of qi deficiency and blood stasis type,enhance the exercise endurance and improve the heart function.
