腹腔镜辅助远端胃癌根治术手术时间延长的影响因素分析

Analysis of Risk Factors for Prolonged Operation Time of Laparoscopic Assisted Distal Gastric Cancer

目的探讨腹腔镜辅助远端胃癌根治术手术时间延长的影响因素。方法回顾性分析行腹腔镜辅助远端胃癌根治术治疗的142例胃癌患者临床资料,按手术时间不同分为长时间组63例和短时间组79例。收集患者资料,分析腹腔镜辅助远端胃癌根治术手术时间延长的影响因素。结果长时间组体质量指数(BMI)≥25 kg/m^(2)、男性、肿瘤分化、消化道重建方式毕Ⅱ、手术经验<100比例高于短时间组,差异有统计学意义(P<0.05);2组年龄、肿瘤部位、合并内科疾病、肿瘤大小、淋巴结转移、浸润深度、淋巴结清扫数量、术中出血量相比,差异无统计学意义(P>0.05)。多因素Logistic回归分析显示,BMI≥25 kg/m^(2)、男性、手术经验<100是腹腔镜辅助远端胃癌根治术手术时间延长的独立影响因素(P<0.05且OR≥1)。结论腹腔镜辅助远端胃癌根治术手术时间延长与性别、BMI和术者经验密切相关,临床需加以重视。

Objective To investigate the risk factors of prolonged operation time of laparoscopic radical surgery for distal gastric cancer.Methods The clinical data of 142 patients with gastric cancer treated by laparoscopic radical resection of distal gastric cancer were retrospectively analyzed,According to the operation time,63 cases were divided into long time group and 79 cases in short time group.Collect patient data,To analyze the risk factors of prolonged operation time of laparoscopic assisted radical resection of distal gastric cancer.Results The ratio of long time group mass index(BMI)≥25 men,tumor differentiation,digestive tract reconstruction mode II,surgical experience<100 was higher than that of short time group,difference was statistically significant(P<0.05);Compared with age,tumor location,associated medical disease,tumor size,lymph node metastasis,depth of invasion,number of lymph node dissection and intraoperative blood loss,(b)The difference was not statistically significant(P>0.05);A multivariate Logistic regression analysis showed,BMI≥25 kg/m2、male,surgical experience<100 were independent risk factors for prolonged laparoscopic-assisted radical gastrectomy for distal gastric cancer(P<0.05 and ORBMI≥1).Conclusion Prolonged laparoscopic radical gastrectomy is closely related to sex,BMI and experience,Clinical attention should be paid to.
